OF OPERATING EXPERIENCE MONTHNEAR: May, 1998 The following chronological sequence by unit is a summary of operating experiences for this month which req.uired .

UNIT ONE:

05/01/98 0000 Unit operating at 100% power, 855 MWe.

05/09/98 1720 Began power reduction to reduce dose rate to facilitate an investigation of a noted increase in reactor coolant leakage.

05/09/98 2112 Stopped power reduction at 50%, 400 MWe.

05/09/98 2304 Began power reduction to place unit at Cold Shutdown as required by Technical Specification 3.1.C.4 due to a non-isolable weld leak on the "C" reactor coolant pump seal injection line.

05/10/98 0151 Generator off-line.

05/10/98 0223 Reactor manually tripped.

05/25/98 1206 Reactor critical.

05/26/98 0032 Generator on-line. Began power increase.

05/26/98 1330 Stopped power increase at 100%, 850 MWe.

05/31/98 2400 Unit operating at 100% power, 850 MWe.

  • DCP 92-22 Design Change Package 05/07/98 Design Change Package 92-22 replaced various older model Unit 1 and Unit 2 solenoid-operated valves (SOV) with newer models. The modification did not affect the original design function or operation of the SOVs.

TM S2-98-02 Temporary Modification 05/12/98 (Safety Evaluation 98-056)

Temporary Modification S2-98-02 replaced the failed manual actuator for service water system isolation valve, 2-SW-478, with a valve handwheel.

Temporary Modification (TM) S1-98-12 installed a cap to isolate the failed capillary tubing on the discharge of the Unit 1 turbine bearing lift pump. Installation of the TM also isolated the pump discharge pressure gauge.

SE 98-060 Safety Evaluation 05/21/98 Safety Evaluation 98-060 was performed to assess the installation of a freeze seal to facilitate the replacement of Unit 1 reactor coolant system valve, 1-RC-6. The evaluation concluded that the installation of the freeze seal in accordance with Mechanical Corrective Maintenance procedure O-MCM-1918-03, "Freeze Seal of Piping," was acceptable.

TM S1-98-13 Temporary Modification 05/24/98 (Safety Evaluation 98-061)

Temporary Modification S1-98-13 installed a recorder on the acoustic monitors used to monitor the performance of the Unit 1 pressurizer safety valves;*

FS 98-10 :uFSAR Change 05/07/98

UFSAR Section 7.2.3.2.7, "Anticipated Transient Without SCRAM (ATWS) Mitigation System Actuation Circuitry (AMSAC)," was revised to clarify the licensing basis associated with the AMSAC C-20 interlock setpoint.

1-0P-RC-011 Operating Procedure 05/11/98 (Safety Evaluation 98-054)

Operating Procedure 1-0P-RC-011, "Pressurizer Relief Tank Operations," was changed to provide instructions for the installation of a mechanical jumper that connects the Unit 1 pressurizer relief tank (PRT) vapor space sample line to the overh.ead gas system. The jumper allows the PRT to be continuously vented to the overhead system without batch releasing to the process vent.

  • GMP-011 General Maintenance Procedure 05/12/98 (Safety Evaluation 98-055)

General Maintenance Procedure GMP-011, "Installation and Removal of Stop Logs," was .

changed to provide instructions for the installation of an electrical jumper that will enable the circulating water intake level alarms to remain functional in the event that one of the intake
