Dear dr. Moseley:
Pursuant to Surry Power Station Technical Specification 6.6.B.1, the Virginia Electric and Power Company hereby reports an abnormal occurrence which was identified on October 17, 1974.
k During Unit No. 2 shutdown for turbine repairs, cracks were found in h
the concrete supporting the "A" Steam Generator (S/G).
Subsequent ultra-(
sonic test (UT) inspection of the steam generator lower ring support swivel f
end coupling indicated a crack. The swivel end coupling was removed and a e
crack was confirmed. Further inspection of the other lower ring support swivel end couplings in Unit No. 2 was undertaken. UT indications are evident on the other coupling "A" S/G and one of the couplings in "C" S/G.
All couplings are being removed to be radiographed and further inspected.
A detail report will be submitted as Report No. A0-S2-74-09 by January 3, 1975.
