Dear Mr. Haynes:

SIGNIFICANT CONSTRUCTION DEFICIENCY DAMAGED BELLOWS ASSEMBLY NO. 2 UNIT, 50-355 HOPE CREEK GENERATING STATION On November 4, 1981, a telephone report was made to Region I representative, Mr. E. Greenman, advising him of damage to a bellows in the Unit #2 primary containment. On December 2, l981, an interim report was submitted which outlined in-vestigative and corrective steps initiated by PSE&G.

The following are the results of those actions:

1. I_n ve_s t i. g a t i o n All other Unit #2 ventline bellows assemblies were inspected for damage. No damage was evident. Addition-ally, " soap bubble" tests at 5 psi and 62 psi were suc-cessfully conducted providing supportive evidence that no similar damage to the remaining Unit #2 bellows as-semblies was substained.

Neither a review of available inspection records nor interviews with cognizant shop and field personnel permitted determination of either the precise cause of the damage or the exact point in time when it occurred.

l The Engineering assessment indicates that the perf ora- l tion was apparently produced thermally as a result of j electrical arcing (probably a we'.d electrode). '

2. Corrective Action No repair or rework activities are planned for the dam-aged bellows assembly in light of PSE&G's cancellation of Unit #2.

All retta ' ning bellows assemblies for both units are equippeo with protective covers. Present sequencing of Unit #1 work activities calls for these protective covers to remain in place up to the start-up phase of plant. construction.
