HOPE CREEK GENERATING STATION DATE:

3 id January 6, 1993 REFT HCT-93-011 Engine driven fuel oil pump, colt P/N P12605391 is being returned for proper configuration. This pump is presently configured for cw engine rotation service. Three (3) pumpa were purchased in 1985 from Colt Industries. Two (2) pumps were corrected on-nite by reversing the plugs, Part Nos. S404974-40 and S404974-41. The plugs were removed from the third pump with some degree of difficulty. A portion of the "O" ring, P/N P12601146, was minsing from plup S404974-40. All pump ports were examined for the missing portion of the "O" ring with no success. The plug was fitted with a new "O" ring and an attempt was made to reinntall the plug.

The plug would only go in about 1/3 of its In consonance with our telephone conversation, the pump is being returnod to Colt Industries for correction of this problem. It is atrongly recommended that the pump be completely disassembled to ensure that the missing part of the "O" ring is not internal to the pump.

In addition, PSE&G has evaluated this deficiency and concluded that this issue is reportable in accordance with 10CFR21. The basis for this conclusion is provided in the attached letter NLR-I92622 dated December 21, 1992.

Colt Industries is requested to perform a complete ovaluation of this concern in accordance with your QA program. Colt industrles should address the scope of this deficiency, root cause and correctivo action taken to prevent recurrence. Colt Industrien is requested to notify other utilities / purchasers that may be affected. A response is requestod no later than January 20, 1993.
