Dear Mr. Haynes:
s SIGNIFICANT CONSTRUCTION DEFICIENCY DAMAGED BELLOWS ASSEMBLY NO. 2 UNIT, 50-355 HOPE CREEK GENERATING STATION On November 4, 1981, a telephone report was made to Region I representative, Mr.
E. Greenman, advising him of damage to a bellows in the Unit #2 primary containment.
The following interim report is submitted in accordance with 10CFR50.55e.
Description of the Deficiency The Hope Creek containment is a " Mark I" drywell and torus with eight (8) vent lines connecting the drywell to the torus.
Each vent line contains two expansion bellows as-semblies.
During testing of the bellows by the installation contractor, Pittsburgh-Des Moines Steel Company (PDM), a hole was detected through both plies of the outer bellows in ventline 8A.
Safety Implications The perforation of both plies of a ventline bellows, if left uncorrected, would result in a breach of containment during accident conditions.
Corrective Action Our Architect Engineer, Bechtel Power Corporation, has in-structed PDM to perform the following:

Inspect all other Unit #2 ventline bellows assemblies for similar damage.

Repair or replace the damaged bellows.
3.
Determine the cause of the perforation and actions nec-essary to prevent recurrence.
We anticipate completion of the inspection and investigation (Items 1 and 3 above) in early January, 1982.
We will re-port the results to you at that time.
