POTENTIAL CONSTRUCTION DEFICIENCY CRD HOUSING WELDS '

HOPE CREEK GENERATING STATION On November 24, 1982, a verbal report was made to Region I, Office of Inspection and Enforcement representative, Mr. W.

Lazarus, advising of a potential significant construction deficiency concerning the use of incorrect weld filler material by General Electric in the root of the welds con-necting the Control Rod Drive Housing to the Reactor Pres-sure Vessel stub tubes. The following interim information is provided in accordance with the requirements of 10CFR50.55(e).

Description of Deficiency:

General Electric I&SE Weld Procedure Specification No.

HC-4009D, Revision 2, used for welding CRD Housings to RPV stub tubes, requires that the root welds be made using the GTAW process and ER NiCr-3 (Inconel) weld wire. The roots of welds No. 14-7, 18-7, 22-7 and a portion of 26-7 were made using 308-L stainless steel weld wire. The discrepancy was noted prior to completion of the fourth root. No further welding was performed on the affected CRD Housings.

Liquid Penetrant Examination, performed on the three com-pleted roots, showed evidence of cracking in each weld examined.

Ronald C. Haynes 12/27/82 Corrective Action:

General Electric I&SE initiated Nonconformance Report No. 49 to document and control the discrepancy. Field Deviation Disposition Report No. KT1-142 was initiated and dispositioned by GE/NEBO. In accordance with FDDR No.

KT1-142, the affected welds were ground out and all weld metal was removed from both the CRD Housings and RPV stub tu be s. The CRD Housings were reinstalled using the correct

' weld filler material.

Additional corrective actions taken by General Electric I&SE to preclude recurrence include:

- Replacement of the wire issue attendant involved,

- Color coding the identification tags on Inconel wire, and

- Providing additional instruction to responsible per-sonnel in weld filler material control.

Based on reporting dates committed to by bechtel and General Electric, we anticipate providing you with a final report by March 31, 1983. This report will address the possible use of incorrect weld wire in previously completed CRD Housings to stub tube welds and an analysis of the safety implica-tions of this deficiency.
