On 11/2/82,'following an ANI Inspector's review of procedure's used regarding main steam line safety valves, it was determined that the "A" safety valv'e had been set with nitrogen at the steam setting of 1240 psig. It was concluded .

that if the valve had been required to relieve steam, the relief'could have.

occurred.above the LSSS specified in T.S. Sec'tiour 2.2.c.

.The valve *had been bench tested at this pressure relief on 3/1/82 and installed using procedure 3.M.4-7. ~The valve is a Cqnsolidated Safety Valve supplied'by Dresser Industrial Valve and Instrument Division, type 3777QA-RT-21 serial g o ' number BK 6262. The high and low steam nameplate settings are 1240 and 1235 - -

psig respectively while the nitrogen settings are 1150 ,and 1125 psig respectively.

Immediately upon learning of the discrepancy, a plant shutdown was initiated.

. Drywell de-inerting commenced and entry was made to remove the valve and replace it with a properly set and tested spare valve. The removed valve will be sent to a testing lab to determine the actual "as-found" steam relief setting.

. Personnel error compounded by procedural inconsistency is the cause of th.is event. . Subsequent to the discovery of the erroneous setting, a maintenance

. department meeting was held that same day. The importance of proper adherence to-written procedures,was stressed, and appropriate. personnel counseled. In addition, in order to preclude a recurrence, a prompt review lEl Procedure 3.M.4-7 was per-formed and changes to the check-off sheet to eliminate the inconsistency were immediately initiated with a complete revision to be made to clarify the intended test procedure.

The unit was placed on line at 0120 on,11/5/82.
