1.0 EXECUTIVE
SUMMARY
This report documents the inservice inspection (ISI), preservice inspection (PSI) and augmented examinations performed at Pilgrim Nuclear Power Station (PNPS) for the period from the end of Refueling Outage 11 to the end of Refueling Outage 12. The examinations performed for ASME Section XI code categories B-D, B-F, B-J and B-K-1 l
are included in the 'first period of the Third Ten Year Inspection Interval of the PNPS ISI Program. Examinations performed for all cier code categories are included as part of the second period of the third inspection interval.
Pilgrim NPS was owned by Boston Edison Company (BECo) during the performance of j
these examinations. Plant ownership was transferred to Entergy Nuclear Generation Company of Plymouth, Massachusetts on July 13,1999.
Tvnes of examinations nerformed:
ASME XI inservice and preservice visual, surface and volumetric examinations of piping, bolting, valves and supports.
. ASME XI subsection IWE visual and ultrasonic examinations of containment per procedure QA 20.03. The General Visual Walkdown of primary containment per procedure PNPS 2.1.8.7 was also performed.
Augmented ultrasonic (UT) examinations for intergranular stress corrosion cracking (IGSCC) per Generic Letter 88-01.
Augmented UT examinations of high energy piping for flow-assisted corrosion (FAC) per Generic Letter 89-08 and Specification M-577.
Visual and ultrasonic examination ofinvessel Core Spray piping in accordance with
- BWRVIP guidelines, in addition to various other invessel visual examinations.
Ultrasonic examination of twenty jet pump beams
.. Augmented UT examinations of Salt Service Water piping spools in accordance with Specification M-591 (Generic Letter 89-13).
Augmented UT examinations of moderate energy piping for flow-assisted corrosion per Specificaton M-619.

'o Examination Details Piping, bolting and valve examinations were conducted in accordance with ASME Section XI 1989 edition, Code Case N-491 for supports and BECo letters to NRC nos.
l 99.035 and 99.066, and NRC letter to BECo 99.063. Letter 99.035 provided the scope of the on line and RFO#12 examinations. Letter 99.066 provided notification of the deferral l
of two Recirc piping weld examinations to the next refueling outage due to a previously l
undocumented interference. Letter 99.063 granted a reduction ofGeneric Letter 88-01 l
inspection frequency for IGSCC Category D nozzle-to-safe end welds from 50% every l
outage to 25% every outage. Containment examinations were performed in accordance with ASME XI 1992 edition with 1992 Addenda subsection IWE as modified by the Code of Federal Regulations. General Electric Company (GE) was the vendor for examinations performed during the refueling outage, while Boston Edison Quality Control personnel performed the on-line examinations prior to RFO#12.

A total of 17 Nonconfonnance Reports were issued and are tabulated in Table 3.
A total of 68 ultrasonic examinations for flow-assisted corrosion effects were performed in accordance with Generic Letter 89-08. One Nonconformance Report and six Problem Reports were written to document reduced wall thickness. All components with reduced wall thickness were analyzed and found acceptable for at least one more cycle of operation.
Ultrasonic wall thickness examinations were performed on eighteen rubber-lined carbon l
steel Salt Service Water (SSW) pipe spools in accordance with Specification M-591 and Generic Letter 8913. Remote visual examination of buried 'A' loop SSW system piping revealed areas with missing and degraded rubber lining as documented on Problem l

Report 99-9207. The inspection was expanded to include buried 'B' loop piping where similar conditions were detected. All degraded conditions were restored and two buried spools were replaced. A total of 21 pipe spools, including the buried spools, were replaced as part of the SSW Pipe Replacement Project during RFO#12.
Three augmented UT examinations of moderate energy piping for flow-assisted corrosion were performed on-line per Specificaton M-619 prior to RFO#12. No degraded conditions were detected. The remainir.g scope of moderate energy piping examinations will be performed during Cycle 13.
Fifty-two system pressure tests for ISI Class 2 and 3 piping were conducted for the first period of the Third Interval ISI Program.
In Vessel Examinations The report of the activities for in-vessel examinations has been provided to the BWRVIP database maintained by EPRI. A summary of the results is provided here:
All examinations cited in BECo letter 99.035 were completed.
Core Spray P8b welds with reported flaws from RFO 1I were re-inspected and the flaws are now attributed to geometry.
All Core Spray P9 welds were examined with no indications fotmd.
Three Core Spray P5 welds that had reported indications in RFO 11 were re-inspected.
One of these is attributed to geometry, one shows no growth, and one indication grew (NCR 99-91) but was found acceptable, i
The shroud vertical welds were visually examined (EVT-1). The goal to achieve coverage of 50% of total weld length was not achieved by a small margin.
All twenty jet pump beams were ultrasonically examined in accordance with current BWRVIP guidelines.
Thejet pump set screw gaps were measured, compared to RFO 11 values, and documented on NCR 99-88. NCR 99-83 documented lack of contact of the wedge with jet pump #16. The canditions were found to be acceptable for continued operation.
The RPV steam dryer had reported indications of erosion (NCR99-71) which were attributed to a hard crud build-up. The deposits were removed.

j Containment Examinations (IWE)
Augmented and program examinations were performed as baseline examinations for the containment IWE examination program in accordance with PNPS procedure QA 20.03 and ASME XI 1992 edition with 1992 Addenda as modified by the Code of Federal Regulations.
The General. Visual Walkdown of primary containment was completed in accordance with PNPS procedure 2.1.8.7 with results documented on Problem Report 99-1145. The l
conditions were found to be acceptable with corrective actions not necessary.
Augmented containment downcomer vent examinations were completed and were the
. subject of NCRs 99-65 and 99-74. Pitting caused by galvanic corrosion which violated minimum wall thickness was reported and evaluated. The evaluation found acceptable thickness remained. Selected areas were recoated to prevent further wall loss. The ~
inspection scope was expanded to include all eight downcomer vents.
- As part of the Augmented Program, the junction of the drywell concrete floor-to-drywell inner surface at the 9 foot elevation was examined for conditions which may allow water i
intrusion (moisture barrier). The examination was documented on NCR 99-50. There is no seal material, nor is a neal specified for the location. As part of the evaluation, three of the areas were selected to have the concrete removed and ultrasonic thickness measurements performed on the drywell wall. In all cases the drywell met originally specified values for wall thickness. The entirejunction was examined completing scope expansion requirements.
. All drywell head bolting was examined and conditions reported on NCR 99-89. The bolting was found acceptable with recommendations for procedural changes regarding lubrication.
Bolting for torus hatch X'-200B was examined and conditions reported on NCR 99-81.
The deformation was not in the area of engagement and was merely a flattening of the i
crowns. No further action was taken.
Examination of the bolting on one of eight Gibbs manways resulted in NCR. 99-73 due to severe thread damage. The Gibbs manways are small, outward-pushing drywell hatches that provide access to the drywell upper stabilizer assemblies. The inspection scope was

expanded to include all eight assemblies which revealed further nonconformances, subsequen@ recorded on NCR 99-80. The condition was determined to have been caused
. during original construction and was evaluated as acceptable since the affected bolting is 4
not stressed when the safety function is required.
