BOSTON EDISON COMPANY PILGRIM NUCLEAR POWER STATION DOCKET NO. 50-293 Attachment to LER 82-008/Ol X-1 j Update Event Description and Probable Consequences On March 31, 1982, while conducting surveillance test 8.I.13, during startup from refuel outage testing, the MO 2301-8 HPCI Injection Valve did not fully open in the required manner. The Priority "A" Maintenance Request was immediately issued to determine the cause. HPCI was declared inoperable, the notification to the NRC via ENS was made.

Cause Description and Corrective Action Investigation showed that a jumper to be used as a bypass of the Limitorque torque switch in the open position was missing. This jitmper was installed, the valve satisfactorily tested and the HPCI system declared operable approxi-mately 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br /> after the discovery of the event.

An Engineering Review, in response to I&E Circular 81-13 was performed; the resulta of which are available for NRC review. "ESR #78 Response (Final)" NED 82-688. The results of the investigation were:

1. 10 MOVs were found not to have the "Open" torque switch bypass feature (jumper) installed. Maintenance personnel installed the jumpers on the 10 valves. The installation of these jumpers made the valves conform to the design prints.
2. ESR #78 recommended no design changes in present valve circuitry.
