Dear Mr. O'Reilly:

As noted in my letter of August 19, 1980, which provided a supplemental response to Infraction No. 413/80-10-01, a reinspection of all completed safety-related support welds >5/16" has been conducted. Review of all completed support packages (approximately 1600) disclosed 54 supports containing a total of 108 welds >5/16" were reinspected by qualified QC welding inspectors.

Results of the reinspection were evaluated by QA; the evaluation included a second reinspection of selected welds. Of the 108 welds reinspected, 101 were found to fully comply with the requirements of the AWS D1.1 Code to which each was fabricated and inspected. Seven welds were found to underrun the specified size requirements by minor amounts. These seven welds were non-conformed on Q-1A 9570 and forwarded to Design Engineering fer evaluation.

Design analysis of the welds in question found all acceptable as welded; minor irregularities in size did not affect the structural intergrity of the welds or the supports.

Prior to this reinspection, welding inspectors did not always measure the size of fillet welds over the full length of each weld. However, that inspection technique did not result in the acceptance of any welds >5/16" that were not structurally acceptable. The importance of inspecting the specified fillet size for the entire length of the weld has been reemphasized. This action was completed September 30, 1980.
