NRC Inspection of Palo Verde Units 1, 2 and 3.
This refers to the inspection conducted by Messrs.
R.
Zimmerman, C. Bosted, G. Fiorelli and J. Ball of this office on April 14, 1986, through May 26, 1986 of activities authorized by NRC License Nos.
NPF-41, NPF-51 and Construction Permit, Number CPPR-143, and by an Enhanced Operations Team led by Mr. Z. Crews during the period of April 21 through April 27, 1986.
Discussions of our findings were held with yourself and other members of your staff on April 29, 1986, and at the conclusion of the inspection.
Areas examined during this inspection are described in the enclosed inspection report.
Within these areas, the inspection consisted of selective examinations of procedures and representative records, inter-views with personnel, and observations by the inspectors.
Based on the results of this inspection, it appears that certain of your activities were not conducted in full compliance with NRC requirements, as set forth in the Notice of Violation, enclosed herewith as Appendix A.
With regard to the problem identified in the Notice of Violation, we are concerned with this example of failure to follow procedures in light of previous findings in this area.
In your response, please address corrective action in the area of procedure adherence broadly.
Two 1985 NRC inspections identified specific concerns regarding procedure adherence; no violations were issued at that time.
Your actions taken as a result of the previous findings dealt effectively with the specific procedural concerns.
However, the current instance of failure to follow a procedure indicates that a broader, more general response to ensuring procedure adherence is appropriate.
