Dear Mrs. Lacal:
On March 31, 2022, the U.S. Nuclear Regulatory Commission (NRC) completed an inspection at Palo Verde Nuclear Generating Station. On April 7, 2022, the NRC inspectors discussed the results of this inspection with you and other members of your staff. The results of this inspection are documented in the enclosed report.
No findings or violations of more than minor significance were identified during this inspection.

SUMMARY
The U.S. Nuclear Regulatory Commission (NRC) continued monitoring the licensees performance by conducting an integrated inspection at Palo Verde Nuclear Generating Station, in accordance with the Reactor Oversight Process. The Reactor Oversight Process is the NRCs program for overseeing the safe operation of commercial nuclear power reactors. Reactor Trip during Plant Protection System Surveillance Testing 71153 Closed
PLANT STATUS
Unit 1 entered the inspection period at full power. On March 27, 2022, the unit began a coastdown for refueling outage 1R23.
Unit 2 entered the inspection period at full power. On January 24, 2022, the unit shut down for short notice outage 2M24A to repair a nitrogen leak on safety injection tank 1A and perform repairs on high pressure feedwater heater 5A. On February 6, 2022, the reactor was made critical following completion of the short notice outage and returned to full power on February 8, 2022, where it remained for the remainder of the inspection period.
Unit 3 operated at or near full power for the duration of the inspection period.
INSPECTION SCOPES
Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection unless otherwise noted. Currently approved IPs with their attached revision histories are located on the public website at http://www.nrc.gov/reading-rm/doc-collections/insp-manual/inspection-procedure/index.html. Samples were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter (IMC) 2515, Light-Water Reactor Inspection Program - Operations Phase. The inspectors performed activities described in IMC 2515, Appendix D, Plant Status, observed risk significant activities, and completed on-site portions of IPs. The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.

The inspectors evaluated system configurations during partial walkdowns of the following systems/trains:
- (1) Unit 1, emergency diesel generator train B during emergency diesel generator train A maintenance outage, on March 4, 2022

- (3) Unit 2, emergency diesel generator train A during emergency diesel generator train B maintenance outage, on March 24, 2022

The inspectors evaluated the following licensee event reports (LERs):
- (1) LER 05000529/2021-001-00, Unit 2, Automatic Actuation of the Reactor Protection System Resulting from a Loss of Reactor Coolant Pumps (ADAMS Accession No.
ML21113A313). The inspectors determined that the cause of the condition described in the LER was not reasonably within the licensees ability to foresee and correct and therefore was not reasonably preventable. No performance deficiency nor violation of NRC requirements was identified.
- (2) LER 05000529/2021-002-01, Unit 2 Reactor Trip During Plant Protection System Surveillance Testing (ADAMS Accession No. ML22032A112). The inspectors reviewed the updated LER submittal and identified no additional performance deficiencies or violations of NRC requirements. The previous LER submittal was reviewed, and an associated violation of NRC requirements was documented in Inspection Report 05000528/2021003, 05000529/2021003, and 05000530/2021003 (ADAMS accession: ML21311A00).
