Concrete Expansion Anchors In response to IE Bulletin No. 79-02, Florida Power Co rporation (FPC)

initiated a test program to identify and correct deficiencies found in anchor bolt installations. The test progran that was initiated was for a random selection of 10% of all supports in the category of having concrete expansion anchor bolts and being Seismic Category I as defined in Procedure IP-102 Rev. O " Inspection of Pipe Snubber Base Plates". This procedure was reviewed on June 13-15, and was found to be inadequate in defining the types of expansion anchors and the parameters of testing of each type of anchor. The NRC inspector held meetings with FPC engineering, plant personnel and Gilbert Associate, Inc., engineers to formulate an acceptable procedure and an acceptable sample size.

-2-On the inspection of June 20-22, the inspector found the procedure was revised to " Inspection of Concrete Expansion Anchors" IP-102, Rev. 2 with a sample size to provide a 95 percent confidence level that less than 5 percent defective anchors are installed in any one Seismic Category I system.

The revised procedure was found to be adequate for inspecting, testing and evaluating the various types of expansion anchors.
