SEQUOYAH NUCLEAR PLANT-(SQN) - NRC INSPECTION REPORT NOS. 50-327, 328/87 INTEGRATED DESIGN INSPECTION (IDI) - REVISED CORRECTIVE ACTION FOR ITEM D3.2-7

References:

1. Letter from NRC to TVA dated November 6, 1987, "Sequoyah i

Unit 2 Integrated Design Inspection (IDI)"

2. TVA letter to NRC dated December 29, 1987, "Sequoyah Nuclear Plant (SQN) - Integrated Design Inspection (IDI): Response to NRC Inspection Report 50-327/87-48 and 50-328/87-48" 3
3. TVA letter to NRC dated Februaiy 14, 1989, "Sequoyah Nuclear Plant (SQN) - NRC Inspection Report Nos. 50-327, 328/87 .

Integrated Design Inspection-(IDI) - Revised Corrective Action to Item D3.2-7" IDI Item D3.2-7, "ERCW Piping Spool Pieces," (Reference 1) involved 13 of.

' ' 22 spool pieces intended to cross-connect various plant fluid systems during postulated flooding conditions that would not fit as specified by design drawings. In the initial response to this item (Reference 2), TVA committed i to refabricate the 13 spool pieces found not to fit and bolt them in place to ensure proper fit before Unit 2 restart. This commitment was completed on - ,

schedule. TVA also initially committed to replace the 22 spool pieces with

' flexible spool pieces, which would remain bolted in place, and to revise the flood protection maintenance Instructions accordingly. Subsequently, TVA identifled a more effective corrective action plan from both a technical and

  • economic standpoint (Reference 3). 1he new corrective action plan included

- the following steps:

1. Identify an enveloping area on either side of the spool pieces in which it

.is believed that any maintenance or modification could adversely affect

, the fitup of the spool pieces.

2. For any supports that fall into the envelope determined by the completion of Item 1 above and are not already engineered supports (i.e., typicals),
  • create and issue engineered support drawings.

3. Revise the support drawings in the areas determined by completion of n Item 1 above to include a cautioning note requiring the review and evaluation of any maintenance or modification of these supports by Nuclear Engineering to ensure no adverse effects on the piping spool piece fitup.
4. Revise the piping physical drawings to clarify the note to state " field _to fabricate to fit" (or similar statement) and to add a note similar to f . Item 3 above cautioning that any modification or maintenance that affects
the physical routing or alignment of piping and components must be reviewed and evaluated by Nuclear Engineering to ensure no adverse effects on the piping spool piece fitup.

i

5. Unit I and common spool pieces will be physically installed to ensure proper fitup and determine if any misalignment has occurred that might be attributable to system creep. This shall be performed by the end of the Unit 1 Cycle 4 refueling outage.
6. Unit 2 spool pieces will be physically installed to ensure proper fitup and determine if any misalignment has occurred that might be attributable to system creep. This shall be performed by the end of the Unit 2 Cycle 4 refueling outage.
7. Unit 1 and common spool pieces will be physically installed.to ensure proper fitup and determine if any misalignment has occurred that might be attributable to system creep. This shall be performed by the end of the Unit 1 Cycle 7 refueling outage.
8. Unit 2 spool pieces will be physically installed to ensure proper fitup and determine if any misalignment has occurred that might be attributable to system creep. This shall be performed by the end of the Unit 2 Cycle 7 refueling outage.

The above first four corrective action steps have been completed. While preparing to perform corrective action Step 5 above, it was discovered that one spool piece designated as " common" needs a Unit 2 refueling outage to be fit up. Consequently, the above long-term corrective steps that remain to be implemented (Steps 5-8) are modified slightly as follows:

5. Unit i spool pieces and those common spool pieces associated with Unit 1  :

will be physically installed to ensure proper fitup and determine if any misalignment has occurred that might be attributable to system creep.

This shall be performed by the end of the Unit 1 Cycle 4 refueling outage.

6. Unit 2 spool pieces and the remaining common spool pieces will be physically installed to ensure proper fitup and determine if any misalignment has occurred that might be attributable to system creep.

This shall be performed by the end of the Unit 2 Cycle 4 refueling outage.

. This shall be performed by the end of the Unit 2 Cycle 7 refueling outage. t The intent.and effect of these corrective action steps are unchanged by this t letter. The only change is to allow the flexibility to schedule the fitup of i the common spool pieces by the endr o either unit refueling outage rather than all by the end of the Unit I refueling outage. .

The existing commitments superseded by this letter and the new commitments ,

made in this letter are stated in the enclosure.
