The' purpose of'this letter is.to notify you of an error we have identified in Lour: response:to the notice of violation contained in Inspection Reports No.

282/89014 Land 306/89015 which was provided-by our letter dated June 5,1989.

.The violation ~in the subject inspection report concerned our failure to

-perform an initial record proof-test prior to the replacement-of maintenance airlock -shaf t seals during the August 1988 Unit 1 refueling outage. .In our response to that violation we stated that the procedures governing local leakage rate testing had been changed to require' retention of as-found and as-1 left test. data.for the airlocks; that statement was incorrect. The subject procedure changes had been initiated at the time of the response.but were not s

. approved.

.This error was the result of miscommunication between the individual i responsible for the preparation of the subject procedure changes and the j

? . individual responsible for the' preparation of the violation response. {

.The procedure changes were initiated during the subject inspection. However, they were not sent through the approval process at that time because of the  ;

large number of other changes to these and other procedures which resulted

from.this inspection'. It was deemed to be more efficient to process all the ,

cprocedure changes which resulted from this inspection as one package. An. ,

additional factor in the decision'to not pursue immediate approval of the /

subject procedure changes was that the procedures will not be used until the next refueling outage for the respective unit. /) l

.This error was identified by our. commitment tracking system. In the process of. documenting the. completion of this commitment, it was learned that the SP  : procedure changes were not approved on June 5,1989 as stated in our response.

The changes to the procedures governing local leakage rate testing were approved on December 29, 1989.to require retention of as-found and as-left test' data for airlocks. We apologize for any inconvenience this error.may

have caused.
