'tViolation #1-
. Technical Specification 3.14.A.2.a requires the licensee to establish,-within one hour, a fire watch to inspect the fire detection zone which has less than the minimum fire detection 1 instrumentation operable.

J0n August 29,.1989,. fire procection Zone 74 was placed in bypacs which rendered nll'of thC Zone 74 fire detection instrumentation inoperable.

- This event was reported as Unit 1 LER 89-013, submitted on September 29, 1989.
.'At 1748'on_ August 29, 1989. with both units at 100% power, a fire drill was g
initiated for a fire zone which encompasses the lower level of the Screen House. At 1752, as part of the fire drill and in accordance with tl.e approved-l fire fighting procedure, a control-room operator sounded the fire alarm,-
- placed the affected fire zone alarm in bypass on the firo protection panel and
'made the proper :.otifications.
l The fire. drill was terminated at 1811. Howevar, the bypass switch for the
-affected fire-zone alarm was not returned to normal following the drill. -The.
alarm remained in bypass and as a result the control room would not have j
received fire alarms from the affected fire zone.
I At' shift turnover.at 2300, an oncoming control room operator trainee failed to note that the; fire zons alarm was still in bypass.
The fire zone alarm panel was not checked by the supervising licensed operator. At the shift turnover at 0700 the~-ext morning, an oncoming control room operator trainee found the-i fire zone-alarm in bypass and returned the bypass switch to the normal
, position.
Corrective Action Taken and Resul.ts Achlaved
'A letter was written to Shift Managers and Shift Supervisors re-emphasizing that traineea.must be closely supervised when performing control room tasks.

. Procedure' changes have been implemented to require the placement of yellow tags on fire 'etection canel bypass switches that are in the bypass position.
)
Pro.edere changes have also been implemented requiring control room log entries following the placement of fire detection zones in bypass.

to' verify that alll Fire Detection Zone bypass switches are returned to the a
normal position following completion of the fire drill.

Technical Specification Table TS. 4.1-1, items #19 and 30 require that automatic =1oad; sequencing and loss of soltage on the 4kv safeguards buses be tested monthly. This is normally accomplished during surveillance testing of the emergency diesel denerators.
D1 Diesel Generatcr can supply Bus 15 for Unit'l and Bus 26'for Unit 2.
Separate procedures, SP1093.1 (D1 Dies el Generator:to Bus 15) and SP1093.2 (D1 Diesel Generator to Bus 26) are each.
performed once a month.
SP1.093.2 was scheduled to be performed on August 22,
-1989._
Whenl preparing to perform the procedure, SP1093.1 was selected from the file-drawer,by mistake, end was performed satisfactorily. On September 5, 1989, when SP1093.1 was scheduled to be performed, operations personnel recalled that SP1093.1 had been performed only two weeks previously.
A review
't of the' Reactor-Logs ~ showed. that SP1093.1 had been perforned on August 22,
- 1989", instead of the scheduled SP1093.2.
Corrective Actions Taken and Results Achieved
