SUBJECT: PRAlRIE ISLAND INSPECTION REPORT 50-282/99004(DRP);

50-306/99004(DRP)

Dear Mr. Wadley:

On May 31,1999, the NRC completed an inspection at your Prairie Island Nuclear Generating

Plant. Tha enclosed report presents the results of that inspection.

Based on the results of this inspection, the NRC determined that a violation of NRC

requirements occurred. This determination was based on Information developed during the

Inspection and provided in Licensee Event Report 1-99-03. The vicietion, identified during the

Updated Safety Analysis Report review project, addressed your engineering staffs identification

of an undocumented modification to several auxiliary building airlock doors between 1977 and

1982 without consideration of the affects on the design basis of the doors. This modification

would have prevented the doors from opening to relieve the pressure assumed in the high-

energy line break (HELB) analysis. The consequences could have been a harsh environment

la an area designed for a mild environment, which could have affected equipment, such as the

safety injection and residual heat removal pumps, needed to mitigate the accident. The doors

were degraded to the point that an extremely complex analysis would be required to determine j

if equipment within the area would be functional following the HELB. Therefore, the failure to

assure the design adequacy of the doors is a violation of Appendix B, Criterion lil," Design I

Control," of 10 CFR Part 50. The NRC has categodzed this violation according to

NUREG-1600, " General Statements of Policy and Procedure for NRC Enforcement Actions

(Enforcement Policy)," at Severity Level Ill. ,

Escalated enforcement was considered for this violation. However, to recognize the efforts of

your staW in identifying problems affecting safety and to give credit for prompt corrective action,

I have been authorized, after consultation with the Director, Office of Enforcement, and the '

Regional Administrator, to exercise enforcement discretion, pursuant to Section Vll.B.3 of the

Enforcement Policy and not issue a Notice of Violation or a Civil Penalty for this old design

issue. The NRC's decision to exercise discretion was based on the follo ving: (1) the violation

was identified as the result of a voluntary initiative to revise the HELB analysis; (2) your staff

implemented prompt and effective compensatory actions; (3) the violation would not likely have

been identified by routine efforts such as normal surveillance and quality assurance activities;

(4) your staffs ongoing design bases reconstitution efforts as well as improved work order and

modification processes should prevent similar occurrences; and (5) this violation is not

reasonably linked to present day performance because the control of modification and design

processes in place today was effective, as documented in Systematic Assessment of Licensee

Performance Report 50-282/98001; 50-306/98001.
