i In response to your letter of June 20, 1989, which transmitted Inspection Reports ] '

No. 50-282/89017 and 50-306/89017, the following information is offered.

i Violation #1  ;

reasons:

The sample location on the day tanks, the level column drain valve, is such that any water or sediment transferred.from the fuel oil storage tanks would~ settle to the bottom of the day tank and be detected.

The current sample location is-conservative with respect to the storage environment. Industry events-and research have shown that fuel

, degradation, biological fouling and contamination are most likely to be detected from a day tank sample.

Obtaining a sample directly from the fuel oil storage tanks is especially difficult in the winter months and does not accurately reflect the condition of the oil at the suction of the fuel oil transfer pump.

Corrective Actions Taken and Results Achieved The new monthly sampling procedures are being prepared which will require that monthly independent samples be taken from each fuel oil storage' tank in addition to the day tanks.

Corrective Actions to Avoid Further Violations The new monthly sampling procedures will be in place by August 15, 1989.

Violation #2 Technical Specification 3.6.A.8 requires, in part, that during maintenance and testing activities, containment integrity is considered intact if the auxiliary building special vent zone (ABSVZ) boundary is opened intermittently, provided such openings are under direct administrative control and can be reduced to less than 10 s.quare feet within 6 minutes following an accident. Operations Procedure D54, Control of Openings in the Auxiliary Building Special Vent Zone Boundary, Rev. 3, requires that a log be kept in the control room which specifies the size and location of all openings in the ABSVZ. The log shall include the time and date openings are made and when they are closed.

This is~a Severity Level IV violation (Supplement 1). '

Corrective' Actions Taken and Results Achieved On April < 12, 1989,- Unit I was at 100% power and Unit 2,was in cold shutdown for refueling. Unit 2 steam piping-in.che Turbine Building had been opened in several places for routine maintenance. When valves in the Auxiliary Building

.were opened for maintenance, paths for air leakage through the Auxiliary Building Special Ventilation Zone boundary were created; the path was through the open steam piping in-the Turbine Building into the Auxiliary Building

-(which is shared by both units) through the open valves. Leakage paths are allowed to exist as long as they are administrative 1y controlled. The potential for creating uncontrolled leakage paths was not recognized by

. persons authorizing the work. When air was found blowing out the valves into the Auxiliary Building, the leakage paths were identified and the openin68

.were placed under administrative control.

' Corrective Actions to Avoid Further Violations Methods to improve administrative control of boundary openings during outages are being developed.

Date When Full Comoliance Will Be Achieved Full compliance will be achieved before the next refueling outage, which is January .1990 for Unit 1.

The event was reported as Unit 1 LER 89-004.
