TheReactorCoolantSystem(RCS)temperature andpressurewereapproximately 60Fand0psigrespectively.

Maintenance personnel wereintheprocessofrepacking motoroperatedvalve(MOV)-738B (CCWInletMOVtoRHRHXB)whichtheybelievedtobeisolated, butwasnot.AtthistimetheControlRoomwasnotifiedofapackingleakfromMOV-738B.Subsequent investigation ofplantpipingandinstrument drawingsrevealedthattheleakwasnon-isolatable.

AnattemptwasmadetoopenMOV-738Belectrically toplacethevalveonthebackseatandstopthepackingleak.Thisattemptcausedtheleakratetoincreasetoapproximately 25gallonsperminuteandMov-738Bwasclosedelectrically.

Atapproximately 2341EST,April2,1991aplantannouncement wasmadetoinformpersonnel oftheComponent CoolingWater(CCW)spillintheAuxiliary BuildingMezzanine Level.Atapproximately 0003EST,April3,1991,refueling operations wereterminated andtheResidualHeatRemoval(RHR)systemandCCWsystemwereremovedfromservicetomanually.placeMOV-738Bonthebackseattostopthepackingleak.Alsoatapproximately 0003EST,April3,1991,theOperations ShiftSupervisor declaredanUnusualEventinaccordance withEPIP1-0,"GinnaStationEventEvaluation andClassification",

EAL:ColdShutdownActivities; Potential LossofCoreCoolingDueToInability ToMaintainResidualHeatRemovalSystemWhileInColdShutdown.

Theseactivities mayincludelossofcoolingtotheRHRsystemsuchasaCCWorservicewatersystems.Alloff-sitenotifications weremadeperEPIP1-5(Notifications).

GinnaTechnical Specifications specifically allowfortheremovalofbothRHRpumpsforuptoone(1)hourprovidedcoreoutlettemperature ismaintained atleast10Fbelowsaturation andnooperations arepermitted thatwouldcausedilution'f thereactorcoolantsystemboronconcentration.

Itisnotedthatalthoughcompliance totheGinnaTechnical Specifications wasbeingmaintained, theOperations ShiftSupervisor conservatively declaredtheUnusualEvent.Thisconservative approachactivated theresponseteamtoprovidesupportshouldconditions furtherdegrade.TheRHRsystemwasthenrunfortwominuteseveryfiveminutesduringthe.isolation ofCCW.

At0027EST,April3,1991,MOV-738Bwasmanuallyplacedonitsbackseatandtheleakwasterminated.

The"A"CCWpumpwasthenstartedandthe"A"RHRpumpwasrestarted.

RHRdischarge temperature wasnot'edtobeapproximately 8Fhigherthanpriortotheevent.-Atapproximately 0122EST,April3,1991,aftertheplantwasreturnedtopre-event conditions andallconcernsresolved, theOperations ShiftSupervisor, withapprovalandconcurrence fromthePlantManager,theSuperintendent

-GinnaProduction, andtheOperations ManagerdeclaredtheUnusualEventterminated inaccordance withEPIP3-4(Emergency Declassification andRecovery).

Alloff-siteagencieswerenotifiedofthetermination oftheUnusualEvent.
