Dear Mr. Haynes:

This is to confirm a phone call from Mr. Dewey Horning, Ginna Shift Supervisor, to your Duty Officer on February 4,1982 at 0310 hours0.00359 days <br />0.0861 hours <br />5.125661e-4 weeks <br />1.17955e-4 months <br />.

This event is reportable in accordance with Technical Specifica-tion Section 6 9 2a (2) " operation in a degraded mode less con-servative than the least conservative limiting condition for operation;" 6.4.2a (4) "... if suberitical, an unplanned reactiv-ity insertion of more than 0.5%

K/K;" and Technical Specifica-tion Section 3.6.lc which states: " Positive reactivity changes shall not be made by rod drive motion or boron dilution whenever containment ic.tegrity is not intact unless the boron concentra-tion is greater than 2,000 ppm".

At approximately 2330 hours0.027 days <br />0.647 hours <br />0.00385 weeks <br />8.86565e-4 months <br /> on February 3, 1982, and contrary to the above, the Reactor Coolant System (RCS) was inadvertently diluted during the performance of T 7P, " Filling and Purge of

'B' Steam Generator" (S/0).

As a result of a procedural inadequacy, only one end of the leaky steam generator tube was plugged and the subsequent transfer of water (

2,300 cals) diluted the RCS fron 1900 ppm to 1684 ppn.

The open end of the S/G tube was plugged and the dilution termin-ated by approximately 0230 hours0.00266 days <br />0.0639 hours <br />3.80291e-4 weeks <br />8.7515e-5 months <br />.

The shutdown margin wa's saintained above 10% during the event.
