Attachment to Close-Out of November 4

1988 Unusual Event On November 4, 1988 at approximately 1038 EDT with the reactor at 100 percent of power, an automatic flow alarm was received in the Control room on S-05 (Main Cable Tunnel Deluge System).

Concurrent with the S-05 alarm, an additional fire alarm Z-36 (Intermediate Building Sub-Basement) was received indicating a potential fire in this area.

At approximately 1039 EDT, the Station Fire Brigade responded to investigate the above fire and flow alarm.

At approximately 1041

EDT, members of.the Station Fire Brigade and,the Fire Brigade Captain arrived at the Cable Tunnel entrance located in the Intermediate building basement.

Initial observations indicated that the Cable Tunnel Deluge system was operating as designed.

Due to h'cavy water volumes exiting the Cable Tunnel in the area of a Z-36 fire detector, it was determined that the Z-36 fire alarm was directly attributed to the actuation of the Cable Tunnel Suppression system and not a fire.

At approximately 1048 EDT the Fire Brigade Captain reported his observations to the Control room and the Operations Shift Supervisors declared an Unusual Event in accordance with SC-100, "Ginna Station Event Evaluation and Classification",

EAL: Fire; Plant fire lasting 10 minutes.

All offsite notifications were made per SC-601, "Unusual Event =Notifications".

Further investigation of this event by the Fire Brigade Captain and the Fire Protection and Safety Coordinator determined that no fire existed in the Cable Tunnel. It was further determined that the Cable Tunnel Deluge system operation was caused by the inadvertent operation of a

manual control station within the Cable Tunnel.

The inadvertent operation of this manual control station was caused when personnel working in the area accidentally bumped the manual control device.

At approximately 1100 EDT November 4,

1988, the Operations Shift Supervisor with approval and concurrence from the Superintendent of Ginna Station and the Fire Protection and Safety Coordinator, declared the Unusual Event terminated in accordance with SC-110, "Ginna Station Event Evaluation for Reducing the Classification".

The rationale involved in the termination was:

There was no actual fire and the actuation of the Cable Tunnel Deluge system was an accidental inadvertent actuation caused by personnel working in the area.
