Further to our notification dated August 27, 1999, this letter has been prcparcd to provide fornial notification of the corrcctivc action performed and to close out the subject 10CFR21 Rcport of Noncompliance.

Description:

The Inovision part 304 connector is a male', non-crimp type BNC plug inteiidcd for the tcrmipation of RG-58/U coaxial cable. Thc military type designation for these'connectors is UG-88/U. Amphenol part mmiber 31-2 or AMP part number 221552-1 connectors may be provided under the 30-4 Inovision part mmiber.

The 30-4 coimector is provided as a sub-coniponent of thc 942-100-4 or 942A-100-4 field termination connector kits. These kits are provided with cvcry 956/956A and 942/942A series of digital mtcmetcrs when the mtemetcrs arc purchased as a spare or rcplaccmcnt indcpcndcnt of a factory prewircd monitoring channel. The instruction manuals provided with the 942 and 956 series mtcmctcrs contain connector manufacturer assembly instructions as part of the vendor data section of thc manual. Additionally, thc 30<

may be incorpomted into a variety of factory-asscmblcd test and operational signal cable assemblies.

These cable assemblics then become sub-assemblics in 960 SRD or 960 LCU, 942WM and other commercial and safety related monitors or displays.

The intended use of these coiuicctors is to provide a removable tertniiiation for low voltage pulse mode or low current signals gcncmted by radiation detectors. Connection points are typically thc rear pmiel of single clianncl mtcmctcrs, 960SF series sealer cards or internal to local cnclosurcs such as NEMA 4 mted junction boxes.

In Junc, 1999, inforniation becanie available tliat indicated that Inovision (nce Victoreen, Inc.) niay lieve accepted an unacceptablc part for thc 30-4 connector. The approved parts for Inovision part number 304 are Amphcnol ¹31-2 and AMP 221552-1. Thc connectors in stock werc rc-inspcctcd and verified to be AMP part number 1-221265. Although recommended by thc manufacturer as suitable for usc with RG-58/U cable, our testing ihdicatcd that thc cable retention propcrtics of thc AMP 1-221265 werc less titan the approved parts. There have bccn no advcrsc reports from the field.

The unacceptable parts iverc removed from stock and scmpped. An investigation on part usage since January, 1997 (thc date that thc unacccptablc part divas received into stock) indicated that four (4) unacceptable parts ntay have bccn procured on safety related purchase orders by Rochester Gas and Electric on tivo (2) sales orders.

Notification was ntadc to Mr. Dave Lovgrcn RGE Quality Assurance by fax and Mr. Al Pitts, RGE Quality Assurance Engineer and Mr. Jeff Wyatt, RGE Purchasing by phone and certified mail. (re. attached)

Ginna Nuclear Power Plant 1503 Lake Road Ontario, NY 14519 Part 21 Notification Re: YourPurchase Order 8's 4500001432 and NQ15875AJW It was recently determined that a shipment of BNC connectors were inadvertently accepted into stock as meeting the specification of Inovision part 30-4. The incorrect connector carries the marking designation 1-221265-0 AhP engraved on its base. This connector may have been supplied with items on the above purchase orders.

Use of the incorrect connector is not believed to pose an immediate serious risk. Our testing indicates that the incorrect connector has a reduced cable retention capacity. The incorrect connector also may not meet the qualifications established for use in nuclear facilities.

A quantity of correct connectors (marked xxxxx UG-SS/U) are on order and expected within the next few days. Four connectors will be sent to you as replacements for those originally supplied on your purchase orders above. Three connectors will replace those supplied on PO 4500001432 and one connector will be sent to replace that supplied on PO NQ15875AJW. Please send your Purchase Order for replacement parts to Ms. Jane Zilka, 6045 Cochran Road, Solon, Ohio 44139 (or FAX 440-349-2307).
