Attachment to Close-Out of December 22 1988 Unusual Event On December 22, 1988, at approximately 0927 EST, with the Reactor at 1004 power, an on-site Medical Emergency was declared due to a reported injured person in the the upper Rad Waste Storage Building.

The station First Aid Team responded and found that a contractor workman had sustained a lower leg injury due to a large filter falling on his leg. At 0935 EST, the First Aid Team requested an ambulance be called.

At approximately 0938 EST, the Operations Shift Supervisor declared an Unusual Event in accordance with SC-100, "Ginna Station Event Evaluation and Classification," EAL: Contaminated Injured; transportation of externally contaminated injured individual to a hospital in that the Shift Supervisor believed the injured person to be potentially contaminated. All offsite notifications were made per SC-601, "Unusual Event Notifications."

As the injured man was found not to be contaminated, the Operations Shift Supervisor, with approval and concurrence from the'lant Manager of Ginna Station, the acting Operations Supervisor, and the duty Engineer, declared the Unusual Event terminated at 1004 EST, December 22, 1988, in accordance with SC-110, "Ginna Station Event Evaluation for Reducing the Classification."

The ambulance left the site with the injured individual at 1004 EST, December 22, 1988.
