III.IDENTIFICATION OFTHEFIRMCONSTRUCTING THEFACILITYORSUPPLYING THEBASICCOMPONENT WHICHFAILSTOCOMPLYORCONTAINSADEFECT:Themotorwasacommercial-grade motormanufactured by:U.S.Electrical MotorsDivisionofEmersonElectricCo.8100W.Florissant Ave.POBox3946St.Louis,MO63136Themotorwasprocuredfrom:AuburnArmature, Inc.48CanogaSt.Auburn,NY13021Note:Thismotorwaspurchased commercial-grade, wascommercially-dedicated byRochester Gas&Electric, andsatisfied alltestingrequirements includedintheprocurement documents.

Page3~IV.NATUREOFTHEDEFECTORFAILURETOCOMPLYANDTHESAFETYHAZARDWHICHISCREATEDORCOULDBECREATEDBYSUCHDEFECTORFAILURETOCOMPLY:A350HPmotorwasinstalled onthe"A"SWpumpinJuly,1996.Post-modification functional testingrevealedunacceptable meggerreadings.

Themotorwasremovedfromthe"A"SWpump,disassembled andinspected.

Theinspection revealeddegradation ofmotorinsulation integrity duetodamagedmotorleads,whichcouldresultinthefailureofthemotorinservice.Theoverallqualityofthemotorwasbelowthatnormallyexpectedforacommercial grademotor.ThesafetyhazardwhichcouldbecreatedbysuchadefectisthecompletelossofServiceWater.Inthescenarioofthe"C"SWpumpoutofserviceandaconcurrent lossofpowertoBus17(whichsuppliespowertothe"B"and"D"SWpumps),alossofallSWwouldoccurifthe"A"SWpumpfailedtooperateduetotheidentified defect.V.THEDATEONWHICHTHEINFORMATION OFSUCHDEFECTORFAILURETOCOMPLYWASOBTAINED:

Theinformation wasobtainedonAugust5,1996.VI.INTHECASEOFABASICCOMPONENT WHICHCONTAINSADEFECTORFAILSTOCOMPLY,THENUMBERANDLOCATIONOF'ALLSUCHCOMPONENTS INUSEAT,SUPPLIEDFOR,ORBEINGSUPPLIEDFORGINNASTATION:Therearefour(4)SWpumpslocatedintheScreenhouse BuildingatGinnaStation.A350HPmotorhadpreviously beenprocuredseparately.

Thismotorwasinstalled toupgradetheexisting300HPmotoronthe"D"SWpumpinAugust,1995.This350motorisofsimilardesigntothe3motorsdiscussed below,andwasalsomanufactured byU.S.Electrical Motors.Thismotorwastestedsatisfactorily andplacedinserviceinAugust,1995,andoperatedtrouble-free.

Asdiscussed below,thismotordidnotcontainadefect.Three(3)350HPmotors,manufactured byU.S.Electrical Motors,werepurchased atalaterdate(asagroup)forupgradeoftheexisting300HPmotorsonthe"A","B",and"C"SWpumps.Asdiscussed below,twoofthesethreemotorswerediscovered tobedefective.

Page4VI~~.THECORRECTIVE ACTIONWHICHHASBEEN,ISBEING,ORWILLBETAKEN;THENAMEOFTHEINDIVIDUAL ORORGANIZATION RESPONSIBLE FORTHEACTION;ANDTHELENGTHOFTIMETHATHASBEENORWILLBETAKENTOCOMPLETETHEACTION:Thefollowing corrective actionshavebeencompleted:

1.GinnaStationMaintenance personnel removedthedefective 350HPmotorfromthe"A"SWpump,andshippedthismotorandoneuninstalled 350HPmotortoanAppendixBmotorfacility.

Thesetwomotorsbothexhibited degradation ofmotorinsulation integrity.

Themotorsweretested,inspected, andconfirmed tobedefective.

Theywererefurbished bythemotorfacility, andreturnedtoGinnaStation.2.The"A"SWpumpremainedinoperable untiloneoftheaboverefurbished motorswasreinstalled andsatisfactorily testedinAugust,1996.3.Thesimilar350HPmotor,installed onthe"D"SWpumpinAugust,1995,hadshownnosignsofmotordegradation.

However,thismotorwasremovedfromthe"D"SWpumpinOctober,1996,andwasreplacedbytheotherrefurbished sparemotor.4.Thus,corrective actionwascompleted forthesetwodefective motorsinAugust,1996,andthesetworefurbished motorsweresubsequently installed onthe"A"and"D"SWpumps,andhavefulfilled operability testrequirements.

5~Electrical testingontheothertwo350HPmotorsdidnotindicateanydegradation ofthemotorinsulation integrity.

Nevertheless, asaconservative measure,theywereshippedtoanAppendixBmotorfacilityinOctober,1996,fordiagnostics andtroubleshooting.

Theseremaining two350HPmotorshavebeendisassembled, inspected, andtestedatanAppendixBmotorfacility.

Electrical testing,performed atthemotorfacility, achievedsatisfactory performance results,confirming thatthemotor(previously installed onthe"D"SWpump)wasoperablewhileinservice.However,someminordeficient conditions werenotedonbothmotors.Thesemotorswerealsorefurbished.

Theserefurbishments areconsidered tobeconservative corrective measureswhichinvolvedmotorreleading, insulation redipandbearingrework.Thefollowing corrective actionsarescheduled forcompletion byMarch,1997:Safety-related motorCommercial GradeItemEngineering Evaluations (CGIEE's) willberevisedtoenhancestepsfortheexamination ofwindingsandoverallworkmanship toensuremotorsareconstructed withacceptable quality.CGIEEpreparation guidelines willbeenhancedtoincorporate thelessonslearnedfromthesedefective motors.
