The facility is the R.E. Ginna Nuclear Power Plant. The basic component is a 350 HP, 1800 RPM, 460 VAC, 3 phase, 500GP Frame, 1.15 SF, vertical hollow shaft, motor for use as a Service Water (SW) pump motor. This component was installed at the R.E.

Ginna Nuclear Power Plant in July, l996, as a modi6cation to upgrade the existing 300 motor for the "A" S%'ump to a 350 HP motor.

III. IDENTIFICATION OF THE FIRM CONSTRUCTING THE FACILITY OR SUPPLYING THE BASIC COMPONENT WHICH FAILS TO COMPLY OR CONTAINS A DEFECT:

The motor was a commercial-grade motor manufactured by:

The motor did not pass post-modification funciiouil testmg, 'The motor was removed from the "A" SW pump, disassembled, and inspected. Inferior worhaanship was "C" SW pump out discovered, most notably damaged motor leads. In the scenario of the of service and a concurrent loss of power to Bus l7, a loss of aH SW mould occur if the "A" SW pump failed to operate due to the identified defect.

V. THE DATE ON WHICH THE INFORMATION OP SUCH DEFECT OR FAILURE TO COMPLY WAS OBTAINED:

The information was obtained on August 5, 1996.

IN THE CASE OP A BASIC COMPONENT WHICH CONTAINS A DEFECT OR FAILS TO COMPLY, THE NUMBER AND LOCATION OF ALL SUCH COMPONENTS IN USE AT, SUPPLIED FOR, QR BEING SUPPLIED FOR GINNA STATION:

There are four (4) FV pumps located in the Screenhouse Buildmg at Ginna Station. The "D" SW pump had its original motor replaced in August, 199S, with a motor manufactured by U.S. Eectrical Motors. Thsmotor was tested satisfactorily at that time and has operated trouble-free since. Three (3) more motors, of similar design to the "D" S% pump motor, were procured by a separate purchase order. Two of these motors were discovered to be defective and have been refurbished. One of the refurbished motors has been installed.

THE CORRECTIVE ACTION WHICH HAS BEEN, IS BKNG, OR WILL BE TAKEN; THE NAME OF THE INDIVIDUALOR ORGANIZATIONRESPONSIBLE FOR THE ACTION; AND THE LENGTH OF TIME THAT HAS BEEN OR %'ILL BE TAKEN TO COMPLETE THE ACTION:

The "A" SW pump remained inoperable untB the defective motor was removed, refurbished, reinstalled, and satisfactorily tested. Thus, corrective action is complete fox the installed motor. Similar corrective actions have been completed for one of the spare motors. The similar motor, installed as the "D" SW pump motor in August, 199S, has shown no signs of motor degradation. However, this motor will be removed and replaced by a refurbished spare motor by November, 1996. The motor (retnoved from the "D" SW pump) and the other spare motor will disassembled and inspected, and refurbished, if necessary, prior to use. Rochester Gas 4 Electric Ginna Statioit maintenance depanment will be responsible for these actions.

Source Surveillance during motor assembly may have detected this defect. Additionally, physical inspection upon receipt could be specified in the purchase order, and completion of commercial dedication could be dependent upon satisfactory 'completion of post-modification functional testing.
