Gentlemen: l i

i We are writing to inform you about a possible defect in certain stainless steel Swagelok l

. tube fittings made from forgmgs.

A list of the part numbers and the U.S. nuclear facilities to which these parts have been j sold is enclosed. All of these facilities have already been notified by us about this j possible problem. The potential defect is limited to the heat code " GAY" and only the l body of the fitting is involved.  !

i The potential defect is a " piping" defect that existed in the bar as it was received from the mill. " Piping" is a small portion of unmelted material that is drawn lengthwise into the ,

bar during the drawing process. The flaw will cause the forging to crack immediately 3 upon pull-up, so if the fitting has been installed and tested without incident, the defect is '

not present. The rate ofincidence among all the parts inspected from the heat is around 0.01%.

We enclose certified material test reports from the supplier of the material, Carpenter \ g Technology Corporation, for each of the sizes made from this heat. We have already instituted an additional inspection procedure for raw material to prevent these type of g2O defects from being accepted by us.
