Mecredy Vice President Ginna Nuclear Production ADDRESS: Rochester Gas and Electric Corporation 89 East Avenue Rochester, New York 14649 IDENTXFXCATXON OF THE FACILXTY, THE ACTXVITY, OR THE BASXC COMPONENT SUPPLIED FOR SUCH FACXLITY WHICH FAILS TO COMPLY OR CONTAINS A DEFECT hf The basic component was the purchased vendor calibration of a "Field Calibration Source" (Victoreen Model 878, Serial No. 109) used to calibrate, among others, Area Radiation Monitoring System (RMS) monitors R-1 through R-9, including R-2 and R-7 (Containment Area Monitors),

and also R-29, R-30 (Containment High Radiation Monitors).

IDENTXFICATION OF THE FIRM CONSTRUCTING THE FACILITY OR SUPPLYING THE BASIC COMPONENT WHXCH FAILS TO COMPLY OR CONTAXNS A DEFECT The vendor calibration of the field calibration source was purchased from:

Victoreen Incorporated 6000 Cochran Road Cleveland, Ohio 44139-3395 NATURE OF THE DEFECT OR FAILURE TO COMPLY AND THE SAFETY HAZARD WHICH IS CREATED OR COULD BE CREATED BY SUCH DEFECT OR FAXLURE TO COMPLY The deviation was that, the field calibration source was sent to Victoreen Incorporated under purchase order NQ-12641-C-JD for required 2 year calibration for two (2) different types of detectors, (1) A Series 877 detector (used for calibration of R-29 and R-30) and, (2) A Series 977 detector (used for calibration of R-1 through R-9). The calibration for the Series 977 detector was performed, but the calibration for the Series 877 detector was not performed. The field calibration source was then sent back to the plant, passed the receipt inspection process, and was accepted for use.

The failure to perform both calibrations was detected by the person on site responsible for using the field calibration source. The field calibration source in question was never used for any calibrations after being accepted by the plant.

The substantial safety hazard that could have been created was that Radiation Monitors are used in support of some Emergency Operating Procedure Responses, such as transitioning to other Emergency Operating Procedures.

Failure of these radiation monitors to be calibrated could have reduced the capability of the operators to mitigate the consequences of an accident.

THE DATE ON WHICH THE INFORMATION OF SUCH DEFECT OR FAXLURE TO COMPLY WAS OBTAXNED The information was obtained on December 10, 1991.

There is one (1) field calibration source at Ginna Nuclear Power Plant that meets the requirement to calibrate Series 877 and 977 detectors. This field calibration source is stored in the Ginna Radio Chemistry Hot Lab when not being used for calibrations in the field.

THE CORRECTXVE ACTION WHICH HAS BEEN, IS BEING, OR WILL BE TAKEN) THE NAME OF THE XNDXVIDUAL OR ORGANIZATION RESPONSIBLE FOR THE ACTION~ AND THE LENGTH OF TIME THAT HAS BEEN OR WXLL BE TAKEN TO COMPLETE THE ACTION Corrective action being taken by Victoreen to assure compliance with future purchase orders has not been identified.

Rochester Gas and Electric is taking the following corrective actions to address the potential for future failure of the vendor to comply with purchase order requirements.

1. The quality of the source surveillance will be improved through:

0 Contact with the ultimate "user" of the material or service. This activity will take place prior to the preparation of a more detailed source surveillance plan to permit the "user" to input any specific or special concerns they may have so that they can be incorporated into the detailed source surveillance plan.

o A technical expert, who has specific knowledge as to what is required, may also be requested to accompany the QA person when performing source surveillance of a vendor.

2. The quality of receipt inspection activities will be improved through the use of a Health Physics representative to review the acceptability of analyses for radioactive source materials.

These corrective actions are being implemented by representatives of the Quality Assurance, Nuclear Assurance and Health Physics groups and will be complete prior to acceptance of the recalibrated field calibration source. This is currently expected to occur during April 1992.

ANY ADVICE RELATED TO THE DEFECT OR FAILURE TO COMPLY ABOUT THE FACILITY ACTIVITY OR BASIC COMPONENT THAT HAS BEEN'S BEINGS OR WILL BE GIVEN TO PURCHASERS OR LICENSEES No further advice is available at this time.
