AZZ -NLI procured and dedicated commercialgrade SBM type switches, manufactured during this time-frame, and subsequently provided to: Exelon -PeachBottom; OPPD -Fort Calhoun; TVA- Watts Bar; KHNP [Korea Hydro & Nuclear Power]."On the basis of our evaluation, it is determined that AZZ -NLI does not have sufficient information to determine ifthe subject condition would, or has, created a Substantial Safety Hazard or would have created a TechnicalSpecification Safety Limit violation as it relates to the subject plant applications."Therefore, purchasers are being advised to perform a visual inspection of the switches at the earliest opportunityto determine if the condition exists."The following information is provided per 1 OCFR 21.21(d) (4). Report of potential failure of GE Hitachi (GEH) SBMtype Switches per 10CFR Part 21Pursuant to 10CFR 21.21 (b), AZZINLI has provided written notification to the affectedlicensees of the potential failure of GEH SBM type switches manufactured (by GEH) in theperiod from January 2012 to December 2014.This condition has been reported in GEH document SC 14-19 (dated Dec. 11, 2014) inwhich GEH notified their affected licensees of the potential failure of SBM type switches,manufactured and shipped January 2012 through December 2014, due to incorrectinstallation of a conical spring (see Figure 1). GEH has attributed the root cause of this springorientation condition to a change in sub-supplier for the contact carrier assembly.This change in sub-supplier occurred in 2011 and GEH received shipment ofswitches with the change in sub supplier no earlier than January 2012.(iii) Identification of the firm constructing or supplying the basic componentwhich fails to comply or contains a defect.Commercial grade GEH SBM type switches procured from December 2012 throughMay 2014 and dedicated for Safety Related use by AZZ I NLI.(iv) Nature of defect or failure to comply and the safety hazard which is created orcould be created by such defect or failure to comply.The cause of the failure was incorrect installation of a conical spring, whichsubsequently shifted and resulted in the switch failure. GEH determined that SBMswitches shipped in the period from January 2012 to December 2014 may have thesubject condition.AZZINLI does not have sufficient information to determine if the subject conditionwould, or has, created a Substantial Safety Hazard or would have created aTechnical Specification Safety Limit violation as it relates to the subject plantapplications.The GEH notification (document Sc 14-19) was not provided to AZZINLI, sinceAZZINLI does not procure SBM type switches from GEH as a basic component.(v) The date on which the information of such defect or failure to comply wasobtained.On December 23, 2014, AZZINLI was initially notified of the GEH documentationregarding the potential failure of SMB type switches via email from an affectedlicensee in receipt of the GEH documentation.(vi) In the case of a basic component which contains a defect or falls to comply, thenumber and location of these components in use at, supplied for beingsupplied for, or may be supplied for, manufactured or being manufactured forone or more facilities or activities subject to the regulations in this part.
