IV.NATUREOFTHEDEFECTORFAILURETOCOMPLYANDTHESAFETYHAZARDWHICHISCREATEDORCOULDBECREATEDBYSUCHDEFECTORFAILURETOCOMPLY:Thenatureofthedefectconsistedofamanufacturingdeficiencyinthereplacementturbochargerforthe"B"EDG.Themachiningofadrainportintheexhaustturbinesideoftheintermediatecasingwasomitted,preventingenginelubricatingoilfrombeingabletodrainbacktotheenginesumpfromtheturbochargershaftandbearings.Therefore,thereplacementturbochargerexhaustturbinesidedrainportdidnotfunctionasthedesignintended.Thisdefectcreatedtheconditionwhereenginelubeoilwasbeingdirectedintocylindersthroughtheexhaustmanifoldpiping.Withoilinthecylinders,theenginecouldhavebeenseriouslydamagedwhenstarted.Thiswouldhaveresultedinalossofredundancyintheon-sitesourcesof480VACpower.However,theEDGwasnotoperableatthetimethedefectwasdiscovered.Thedeficiencywasfirstindicatedwhenthepre-lubesystemwasinoperation.The"B"EDGwasbeingpreparedforpost-maintenancetestingonMarch29,1999,whiletheplantwasdefueled.Noactualsafetyhazardexisted,becausethepost-maintenancetestinghadnotbeenperformed.V.THEDATEONWHICHTHEINFORMATIONOFSUCHDEFECTORFAILURETOCOMPLYWASOBTAINED:TheinformationwasobtainedonMarch30,1999,followingdisasssemblyoftheturbocharger.VI.INTHECASEOFABASICCOMPONENTWHICHCONTAINSADEFECTORFAILSTOCOMPLY,THENUMBERANDLOCATIONOFALLSUCHCOMPONENTSINUSEATgSUPPLIEDFORgORBEINGSUPPLIEDFORGINNASTATION:Thereare2(two),permanentlyinstalledEDGsatGinnaStation.Onlythe"B"EDGwasprovidedwithareplacementturbocharger,whichwasfoundtocontainthemanufacturingdeficiencystatedinIV.above.Theturbochargeronthe"A"EDGwasnotreplaced,anddoesnotcontainthestateddeficiency.

VII~THECORRECTIVEACTIONWHICHHASBEEN~XSBEXNGgORWILLBETAKEN;THENAMEOFTHEINDXVIDUALORORGANIZATIONRESPONSIBLEFORTHEACTION;ANDTHELENGTHOFTIMETHATHASBEENORWILLBETAKENTOCOMPLETETHEACTION:Thecorrectiveactiontakenwastoremovethenewreplacementturbochargerfromthe"B"EDGanddisassembleandinspectit.Duringdisassembly,itwasdiscoveredthattheintendeddrainporthadnotbeenmachinedintotheintermediatecasing.Underthesupervisionofthemanufacturer'srepresentative,theintermediatecasingwasreworkedtooriginalrequirements,openingthedrainpath.Theturbochargerwasreassembledandreinstalledonthe"B"EDG.Post-maintenanceandoperabilitytestingwassuccessfullyperformedandthe"B"EDGdeclaredoperableonApril2,1999.PersonnelfromRochesterGasandElectricNuclearEngineeringServicesandfromGinnaStationMaintenanceDepartmentwereresponsibleforthecorrectiveaction,withtheassistanceofthemanufacturer'srepresentative.CorrectiveactionswerecompletedonApril2,1999.VIII.ANYADVICERELATEDTOTHEDEFECTORFAILURETOCOMPLYABOUTTHEFACILITY'CTIVITY'RBASICCOMPONENTTHATHASBEEN'SBEING,ORWILLBEGIVENTOPURCHASERSORLICENSEES:Amissingoildrainportcannotbeobservedwithoutdisassemblyoftheturbocharger.Innuclearserviceapplications,theuseofaColtecIndustriesModel730turbochargerontheALCO251engineisuniquetoGinnaStation.OrganizationsprocuringorinstallingturbochargerssuppliedbyColtecIndustriesunderanAppendixBprogramshouldconsiderthepotentialforthisdeficiency.ThisissuewillbediscussedatafuturemeetingoftheALCOOwners'roup.
