1 By letter of July 6,1987 the Annual Diesel Generator Reliability Data Report for 1986 was submitted in accordance with Technical Specification 6.9.1.12.

This report pre"ided ths number o'. tests (valid or invalid) i and the number of f ailures for esch diesel generator at Far1r:y Nuciear r

Plant for 1986.

Also provided was the information identified in Ragulatory Pesition C.3.b of Regulatory Guide 1.108. Revision 1, 1977 1

for each failure. During an inspection of November 2-6, 1987, the NRC stated that both valid and invalid emergency diesel generator tests and f-\\ lures should be included in test data logs.

Attached is Revision 1 i

0 The following paragraph provides the information identified in Regulatory Position C.3.b of Regulatory Guide 1.108. Revision 1, 1977 for the valid l

failure identified above.

2C DG Failure on 5-12-86 l

t At 1350 on 5-12-86, the 2C DG.as started to perform an operability test.

Upon starting, the plant operator noticed that no frequency or voltage was indicated.

However, speed indication and DG status lights functioned correctly. The control power fuses and control circuits vere check-d and no problem vas found.

The DG vas restarted and it tripped due to overspeed. No cause for the trip was discovered. The overspeed trip switch was reset. The field flashing circuit was checked and the K1 field flashing relay was manually operated. The DG vas restarted and proper Indications were received locally and on the eaergency power board. The DG t

va-returned to service at 1955 on 5-12-86.

The K1 relay was checked again late. in the shift and was verified to be operating properly. No correlation between the overspeed trip and loss of indication could be found.

This was the first failure in the last 100 starts for this type of DG, Na change in the surveillance schedule was necessary. The surveillance interval remaine' at fourteen days in accordance with Technical Specification Table 4.8.1.
