Farley Nuclear Plant NRC Emergency Planning Weaknesses Identified In Inspection Report Nos. 50-348/89-32 and 50-364/89-32 By letter of February 1, 1990, the NRC identified four emergency exercise weaknesses in the areas of offsite notifications, re-entry team control, connunications with re-entry teams, and protective action recommendations.

These weaknesses have been reviewed and the following actions have been taken.

'Ihese actions are consistent with our proposed actions as described in Paragraph 13 of the subject report.

1. ' Exercise-Weakness 50-348, 364/89-32-02: Failure to make offsite notifications of emergency declarations in a manner consistent with procedure FNP-0-EIP-26 (Paragraph 6).

APCo Response FNP-0-EIP-26 has been revised to require the Initial Notification Message to be filled in, and read or transmitted to the appropriate agency for-initial notification of an emergency condition or initial report of.

upgrade / escalation of an emergency classification. Shift Supervisors and Emergency Directors have been instructed to use the Initial Notification Message for all emergency classification declarations and upgrades.

2. Exercise Weakness 50-348,364/89-32-03: Failure to brief, control and track 2 of 7 re-entry teams.as specified by procedure FNP-0-EIP-14 (Paragraph

'7.C).

APCo Response FNP-0-EIP-14 has been revised to provide more specific instructions for

-the control of re-entry teams and to provide improved controls regarding transfer of' personnel between assembly areas. On-call TSC personnel responsible for re-entry teams were trained on the procedure revision and ,

were instructed to consider re-entry requirements'for personnel who leave an operational Support Center (OSC).

3. Exercise Weakness 50-348,364/89-32-05: Failure to provide adequate means for. plant re-entry teams to comunicate with the TSC (Paragraph 3).

APCo Response FNP-0-EIP-14 has been revised to ensure adequate means for re-entry teams to comunicate with the TSC are available and to address actions to be

• taken if comunication cannot be established. On-call TSC personnel responsible for re-entry teams were briefed on the procedure revision.

4.- Exercise weakness 50-348,364/89-32-06: Failure to provide a timely and appropriate PAR to cognizant offsite authorities following the General Emergency declaration (Paragraph 10).

trained on this issue and were instructed that partial zone evacuations are not allowed. FNP-0-EIP-26 has been revised to highlight the requirement that Protective Action Recommendations must be made at the time of initial notification or upgrade / escalation to General Emergency.

The effectiveness of the corrective measures outlined above will be evaluated by one of several methods, i.e., table top exercises, practice exercises, and/or training examinations. Table top exercises and small scale TSC drills have been performed in February and others are scheduled. By March, each on call crew will have participated in at least one training exercise. Plant wide drills' concentrating on TSC and EOF functions are scheduled for August 1990. Further practice exercises will. incorporate coordination between State agencies in September and October 1990. Discussions with state agencies are ongoing to mutually resolve problems identified in'the 1989 exercise. Direct

(both initial and upgrades) is being implemented, along with additional measures to ensure adequate comunications with each agency. Training programs for site and corporate personnel will be strengthened and emphasis will be l placed on corrections of self-identified findings from the previous exercise.
