O Violation Technical Specification 6.4.1.g, requires the station shall be operated and maintained in accordance with approved Radiation Control Procedures. Station Directive 3.8.2.3, " Health Physics High Radiation Door Tour" 12/6/79.

Section 1.1, states all high radiation area doors are securely locked or guarded at all times except for entry and exit.

Contrary to the above, on May 13, 1982, the door providing access to high radiation areas in Room 304 (Radwaste Compacting Area) elevation 796 was found unlocked. Positive access control over each individual entry into the room was not in effect.

This is a Severity Level V Violation (Supplement E.2).

Response

1) Admission or denial of the alleged violation:

This violation is correct as stated.

2) Reasons for the violation:

The door closure device for the cited door is apparently not strong enough to completely shut the door under certain conditions. This occurs when the truck bay roll-up door in the same general area is open, creating a draft through the entrance to Room 304. Although the door to Room 304 will appear to be closed under these conditions, it is not securely latched. Die to the number of personnel entering and exiting through that door on : he day of the violation, it cannot be positively determined who failed to assure that the door was latched after passage.

3) Corrective actions taken and results:

The door was fully closed and latched after identification of the violation. The general station HP training and appropriate station directives have emphasized and will continue to emphasize the requirements j for positive control of access to high radiation areas.

4) Corrective actions to be taken to avoid further violations:

More powerful door closure devices will be installed on the doors l to Room 304 and, if needed, on other High Radiation Area doors. This will assure complete closing and latching of the doors under any i conditions. The Preventative Maintenance program for all High Radiation Area doors will be re-evaluated and revised if necessary to assure that these doors are self-latching under any conditions.

l l 5) Date when full compliance will be achieved:

i The door closure devices for Room 304 and other doors, if needed, will I

be replaced by September 15, 1982. The re-evaluation and any subsequent revisions to the PM program for High Radiation Area doors will be completed i by December 15, 1982.
