Watson, of your of fice, of a Reportable Deficiency at the Graal Gulf Nuclear Station (GGNS). The deficiency cor.cerns failure of the internal coating of the ADS, MSRV, MSIV accumulators. This deficiency af fects thirty-six (36)

Unit 1 accumulators. The condition has been determined to be reportable under the guidelines of 10CFR21 for Unit 1.

Investigation into the applicability of this condition to Unit 2 is c ont inuing . We expect to submit a Final Report by May 31, 1984.

The Main Steam Isolation Valve (MSIV) and Main Steam Safety Relief Valve (MSRV) accumulators were manufactured by the Buf falo Tank Division of Bethlehem Steel Corporation and supplied to Grand Gulf by the Bechtel Power Corporation, Gaithersburg, Maryland.

4. Nature of the deficiency . . . and the safety hazard which . . . could be created by such a deficiency ...:

A. Description of the Deficiency The deficiency concerns' the failure of the internal coating applied to the MSIV and MSRV valve accumulators. The coating application on the accumulators was not ad eq ua t e.

The results of " air blows" conducted as a result of an earlier defic-iency of the instrument air system prompted an internal inspect ion of the coated carbon steel accumulators and ADS receivers. This inspec-tion revealed failures in the internal coating of the accumulators.

However, the internal coating on the four (4) ADS receivers was s atis facto ry. The failure of the coating was exhibited by blister-ing, peeling and uncoated areas Which resulted in rust fo rmat ion inside the vessels. It has been detennined that the cause of failure was due to improper surface preparation and/or application of the coating.

B. Analysis of Safety Implications The existence of uncoated carbon steel vessels could result in corrosion products being introduced 'into the air supply to the MSIV's, ADS and Safety / Relief valves which could impair the

the MSIV solenoid valves 1f rom changing position, when required, during a steam. line break. This _ vould prevent closure of the MSIV's which could lead to exceeding the' limits of 10CFR100. -

Mississippi Power and Light received information of the deficiency on July 2, ~1983. We reported the deficiency to Ms. L. Watson, of your office, as a Reportable Deficiency for Unit 1 on November 15,'1983. An evaluation for Part 21 has been completed -for. Unit 1 and the MP&L "Res ponsible

Of ficer," Mr. - J. P. McGaughy, Jr. , will be notified when he returns to his office.

The thirty-six (36) accumulators are located at the Grand' Gulf Nuclear

. Station, Unit 1.-

It is not known at this time .if this deficiency af fects Unit 2.

We do not have knowledge of the location of defective equipment located other than at Grand Gulf Nuclear Station.

A. Corrective Actions Tanen All thirty-six (36) Unit 'l coated carbon ~ steel accumulators supplying

the MSIV's, ADS and l Safe ty/ Relief valves have been repaired. The

~

vessel internals were vacublasted and recoated with Ameron Amercoat

90. E1cometer adhesion tests were conducted on, sample discs coated with Ameron Amercoat 90 with resultant values larger than those required by , ANSI N5.12-1974, " Protective Coating's (paints) for the Nuclear Industry".

Applicability and action required for Unit 2 is being deteneined at this time.
