le ENCLOSURE NOTICE OF VIOLATION Duke Power Company . Docket No. 50-370 McGuire Unit 2 License No. NPF-17 During the Nuclear Regulator Commission (NRC) inspection conducted on October 21

- November 20, 1986, a violation of ' NRC requirements was identified. The violation involved a failure to comply with Technical Specification (TS) 6.8.1 in that written procedures were not properly implemented. In accordance with the

" General Statement of Policy and Procedure for NRC Enforcement Actions", .10 CFR Part 2, Appendix C (1986), the violation is listed below: .

Technical Specification 6.8.1 requires that written procedures be esta-blished, implemented, and maintained covering the activities recommended in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978.

Regulatory Guide 1.33, Revision 2, February 1978 recommends that general .

plant operations including heatup from cold shutdown to hot standby be covered by written procedures.

Step 12 of Enclosure 4.2, " Pre-Heatup Checklist", of McGuire Operating Procedure OP/2/A/6100/01, " Controlling Procedure for Unit Startup", directs that no-load programmed steam generator levels be established per OP/2/A/6250/03A, " Steam Generator Wet layup Recirculation".

Contrary to the above, OP/2/A/6100/01 and OP/2/A/6250/03A were not properly implemented during a Unit 2 startup on November 17, 1986, in that a control room operator initialed Step 12 of OP/2/A/6100/01 as complete, when, in actuality, steam generator levels had not been established per OP/2/A/6250/03A. This resulted in a lifting of the steam generator wet layup pump relief valves and a spill of approximately 3000 gallons of secondary water to the auxiliary building floar.

This is a Severity Level IV (Supplement 1) violation and applies to Unit 2 only.

Pursuant to the provisions of 10 CFR 2.201, Duke Power Company is hereby required to submit to this office within 30 days of the date of the letter transmitting
