l NOTICE OF VIOLATION i i

Duke Power Company Docket No. 370 I McGuire Unit 2 License No. NPF-17 During an Nuclear Regulatory Commission (NRC) inspection conducted on ,

September 26, 1993, through October 16, 1993, a violation of NRC requirements i was identified. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, the violation is listed below:

Technical Specification 6.8.1 requires that written procedures be 1 established, implemented and maintained covering the applicable procedures recommended in Appendix A of Regulatory Guide 1.33, Revision 2, February,1978, which includes the operation of the letdown charging i system. i Contrary to the above, the abnormal operating procedure AP/2/A/5500/12, Loss of Normal letdown Charging or Seal Injection Flow procedure, was implemented on September 9, 1993, when the unit, while operating in Mode 3, experienced a loss of letdown flow due to a system isolation valve ,

2NV-2 going closed for no apparent reason. The procedure was inadequate in that it failed to provide any cautions, instructions, or guidance and it failed to require any special controls to alert the operator of the potential voiding of the letdown piping when 2NV-2 closes. This inadequacy resulted in the formation of an undetected void in the letdown system when 2NV-2 drifted shut because its valve interlock did not cause the downstream letdown orifice isolation valve 2NV-458 to simultaneously close. Later, 2NV-2 was cycled opened, causing the system to experience a water hammer that contributed to a letdown system leak of about 10-15 gallons per minute to occur inside the containment building.

This is a Severity Level IV (Supplement I) violation.

Pursuant to the provisions of 10 CFR 2.201, Duke Power Company is hereby required to submit a written statement or explanation to the U.S. Nuclear i Regulatory Commission, ATTN: Document Control Desk, Washington, D.C. 20555 i with a copy to the Regional Administrator, Region II, a copy to the NRC  :

Resident Inspector, at the facility that is the subject of this Notice, within l 30 days of the date of the letter transmitting this Notice of Violation  !

(Notice). This reply should be clearly marked as a " Reply to a Notice of-  !

Violation" and should include for each violation: (1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the i corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the  !

date when full compliance will be achieved. If an adequate reply is not-
