FORT CALHOUN STATION UNIT NO. 1 Emergency Plan Implementing Procedure EPIP-OSC-2 Emergency Plan Activation Method for initiating the Emergency Plan from the Operations Control Room after the Emergency Classification has been completed.

I. PURPOSE This procedure provides the instruction to be followed by the Shift Supervisce or his designee when an emergency occurs that requires initiation of the emergency plan.

II. PREREQUISITE A. The emergency has been place.d into one of four (4) classifications.

B. An initial action has been taken to ensure the plant is in a safe, possibly shutdown, condition.

III. PRECAUTIONS A. All significant events and action shall be logged in the Operation Log book.

B. The person relieving the Shift Supervisor as Emergency Duty Officer shall be fully briefed prior to assuming the responsibility.

IV. PROCEDURE

1. Notify plant personnel.

Notification of Unusual Events: Announce the classification condition twice over the Gaitronic system. Consider evacuation of compartments within the containment and auxiliary buildings.

Alert, Site Area, General: Sound the Nuclear Emergency Alarm followed by an announcement on the Gaitronic with special evacuation instruction. Repeat instruction.

2. Notify the Manager - Fort Calhoun Station or his designee. The Manager - Fort Calhoun Station or his designee will then notify 0.P.P.D. management of emergency class and plant status.
3. Initiate the " Emergency Call List" by contacting Technical Augmentation Staff Member Emergency Team Tag #1/1A and assign the duty for contacting the OPPD response personnel required. For every emergency classification, this individual is directed to notify members of the Technical Augumenta- tion Staff and those members of the Emergency Team who are required to augment the operating shift. For the Alert emergency classification and greater this individual sh '_1 notify those members of the Emergency Team who were not previously notified.

Alert: The individual reporting for Notification of Unusual " Event emergency will also report for an Alert emergency. The Operations Support and Technical Support Groups will be acti-vated. As needed, additional members of the Operations Support Group will be activated by the Plant Operations Manager. The ED0/ Recovery Manager will notify the Administrative Logistics Manager of the emergency classification and status. The Admini-strative Logistics Manager will notify the Technical Support Manager to initiate activation of the Technical Support Group; and will alert the remaining members of the Recovery Organization to a " standby" status using EPIP-RR-1. The Technical Support Manager will notify the following personnel: Licensing Administrator Security & Technical Support System Analysi's/ Procedure Administrative Supervisor Support Coordinator Procedure / Training Shift Support Coordinator Supervisor Instrument & Control Support

  • Coordinator Emergency Team Tag 1/1A will notify the remaining members of the .

Emergency Team to report for assignment (s). The remaining members of the Recovery Organization will be activated when one of the following is met:

a. The Recovery Manager, Emergency Coordinator, Plant Operations Manager, and the Emergency Duty Officer agree that the Recovery Organization should be activated for possible emergency escala-tion. Activation will be made in accordance with EPIP-RR-1,
                                          " Activation of Recovery Organization".
b. The event is elevated to the next higher emergency classifica-tion.

Site Area and General: Those individuals reporting for the Alert emergency will also report for a Site Area and/or General Emergency. The remaining members of the Recovery Organization will be notified to report to their assigned emergency response center as soon as possible, in accordance with EPIP-RR-1, "Acti-vation of Recovery Organization".

4. Notify State / local support groups.

b. The caller will identify himself and ask for the name of the
  • NSP dispatcher. For verification purposes the caller will respond to the NSP dispatcher with the badge number of the dispatcher using the Personnel Listing, Letter dated May 13, i 1981. Controlled copies of this confidential listing are I maintained in the Fort Calhoun Station Control Room Key Locker.
c. If further authentication is deemed necessary the caller may l also provide the initials of the dispatcher provided on the
  • Personnel Listing.
d. If the verification method has not been successful, another 3 NSP dispatcher should be contacted to accomplish verification l with the caller. j
e. The alternate method of notification is the National Warning System (NAWAS). Verification is not required for this method.

6. Notify the NRC - Region IV using dial type' phone's. _ ; and the NRC's Operations Center by direct red telephone network. This must be accomplished within one (1) hour. (Refer to OPPD - Standing Order R-11).
7. Provide initial technical information to Nebraska State Civil Defense Agency and Iowa Office of Disaster Services by telephone. Complete the form shown in Appendix 2 of this procedure for an emergency classified as " Notification of Unusual Event". The form shown in Appendix 3 should be used for emergency classes fo " Alert", " Site Area" and " General".

NOTE: Steps 8 thru 12 apply to emergency classes greater than the Notification of Unusual Event classification.

8. Complete accountability of Operations personnel. Initiate personnel search and rescue as needed.
9. Direct in plant radiological survey.
10. Direct off-site surveys, as additional emergency monitors arrive.
11. Until such time as the EOF is activated, prepare and transmit emergency update information to the State Emergency Operations Centers and State

() Forward Posts. The form shown in Appendix 1 of EPIP-EOF-7 entitled

              " Nuclear Power Plant Incident Update Report to Offsite Government Agencies" will be used to prepare information for transmittal.
12. Transfer Emergency Duty Officer (EDO) responsibility with complete briefing after EOF is established. The transfer shall be performed in accordance with either transfer shall be performed in accordance with either EPIP-EOF-13 (" Shift Supervisor /EDO to EDO Transition")

D. Retain all samples. Clearly label each sample with collection location, date time and name of person performing the survey (s). Also note the volume of air sampled for air samples. Store each sample in individual.., . plastic bag. . E. When using the radio for communications speak slowly and clearly. (If communications should fail. nne. member of the team should call the Monitor Coordinator ss,__ or return to the EOF for additional communications equipment.) . F. Maintain instruments in service during transit in order to identify trends. G. Use Channel #1 to communicate with the Radio Operator at the Emergency Operations Facility unless instructed otherwise by the Monitor Coordinat.or. H. Count all samples in;1ow background area. This may necessitate leaving the plume area. IV. PROCEDURE

1. Assemble at the Emergency Operations Facility (EOF).
2. One member of the team must pickup the keys to the designated emergency monitor vehicle which is parked on the east side of the EOF. n.

(NOTE: Keys are located behind the assignment tag in the EOF.) " -

3. Place offsite emergency kit and additional equipment / supplies (as listed under II.F of this procedure) into the monitor vehicle or verify that they are already in place.
4. Connect the SAM-2 to its power supply, and turn the instrument on.

Plug in the RADECO H809V and verify its operation.

5. Obtain initial instructions from the Monitor coordinator for sample type and location for first monitor station.
6. Locate monitor station using the " Emergency Radiological Monitor Station Bookle t"; and proceed to the monitor station as described in the directions of the booklet.
7. Begin surveying for radiation dose rate prior, to entering vicinity of monitor station. Determine dose rates with either an E-120 with G-M probe or a R0-4 Ion Chamber meter. Record location (s) and dose rate reading (s) into log, Figure 18.1 of this procedure.
8. Report significant dose rate changes to the Monitor Coordinator through the Radio Operator.
9. Coordinate locating the plume's length, width and intensity with' the
              -Monitor Coordinator.

I. PROCEDURE

(Continued) Prepare SAM-2 for counting iodine cartridge samples, as outlined in 10. EPIP-EOF-3, Section IV.2.c.(1)

11. Setup E-120 with HP-190 probe for counting particulate air samples and smears, as outlined in EPIP-EOF-3, Section IV.2.a.
12. Pull and analyze air samples as directed:
a. Collect the radioiodine and particulate sample (s) using a Radeco air sampler in the manner described in EPIP-EOF-3, Section IV.2.c.(2).
b. Analyze the radioiodine sample using the SAM-2 analyzer, in accordance with EPIP-EOF-3, Section IV.2.c.(3).
c. Analyze the particulate air sample using the E-120 with HP-190 probe, in accordance with EPIP-EOF-3, Section IV.2.a.
13. Smear flat surfaces of 100 cm2 area for each smear.
14. Analyze smears using E-120 with HP-190 probe, as outlined in EPIP-EOF-3, '

15. Log all results. (Samples of survey logs are provided in Figures 18.1, 18.2 and 18.3.)
16. Report all results to the Monitor Coordinator through the Radio Operator.
17. Continue surveying for radiation dose rate trends.
18. Proceed to next sample station (s) in accordance with Monitor Coordinator directions. Identify insturment fluctuations while travelling. At next monitor station, repeat procedure Steps 12 through 17.
19. Continue performing radiological surveys (i.e. air samples, smears, dose rate surveys) until the full plume is defined and the team is instructed to return to the EOF.
20. Upon returning to the site, remove all protective clothing and contact the gate monitor for a through frisk. This should be done on the west side of the EOF, to facilitate decontamination, if necessary.
21. Deliver all samples and logs to the Monitor Coordinator af ter returning to the EOF.
