FORT CALHOUN STATION PLASTIC LABEL USAGE 1.0 PURPOSE The purpose of this OPD is to provide direction to Ops Department personnel on the use of plastic labels for control switches in the Control Room and out in the plant.

2.0 APPLICABILITY This OPD applies to all Operations Department personnel.

3.0 DISCUSSION We have been using plastic labels on Control Switches during outages and on line in order to identify significant plant conditions that all operators should be aware of. These conditions / lineups are:

- RCS Makeup Water flowpath

- RCS Boric Acid flowpath

- Shutdown Cooling (SDC) flowpath

- Diesel Generator status related to backup power for SDC 0- - Mode 5 PeerChecks and

- Overpressure Protection.

- Out of-Normal Switch positions 4

The intent of using the plastic labels during outages is two-fold:

1.

To eliminate the need to caution tag the designated Boric Acid or Makeup flowpaths. In the past, this practice caused unnecessary delays when the flowpaths had to be changed and the caution tags re-written and re hung and,

- 2.

To make it easier for the operators to determine what the designated flowpaths are by using a visual cue. (when using caution tags, we typically had numerous tags on the same or related components and it was difficult to determine exactly what the designated flowpadi was).

The intent of using the plastic labels on-line is to:

= li To provide a visual crew to'the Operating Crew when a switch is not in its nonnal

' position. His will help ensure system operability is maintained during all ionditions.

In order to support the usage of the plastic labels DURING OUTAGES, the follow guidelines are established for each type oflabel:

1, RCS Makeue Water Flownath; Tags will be hung as required when directed by the procedure. These tags are green circles and will be hung on control room

- valve control switches, pump control switches and manual valves. Components that have the green labels installed will be designated on form FC-1291 which is maintained in a notebook kept near the LSO's workstation. In addition, the designated flowpath will be indicated using colored markers on the large RCS/SI/CVCS P&ID (kept in the Control Room and the OCC).

2.

RCS Boric Acid Flownath Tags will be hung as required when directed by the procedure. These tags are tan octagons and will be hung on control room valve control switches, pump control switches and manual valves. Components that have the tan labels installed will be designated on form FC-1290 which is '

maintained in a notebook kept near the LSO's workstation. In addition, the designated Gowpath will be indicated using colored markers on the large -

RCS/SI/CyCS P&ID (kept in the Control Room and the OCC).

Shutdown Coolino Flownath: Tags will be hung as required when directed by the O procedure. These tags are blue squares and will be hung on control reom valve control switches, pump control switches and manual valves, here is currently no reouirement to annotate tag installation using a form for the SDC flowpath. Since some of the controls for the SDC system are locatei on the vertical section of CB-1,2,3, and don't have a pistol grip control switch to hang a tag on, small magnetic SDC labels have been provided that should be placed adjacent to these controls.

DG Status for SDC Rdun Powq These are large, fluorescent orange labels that will be hung on the front of the appropriate DG panel as directed by the procedure.

Mode 5 Peer Cheh These tags are orange triangles and will k. hung on

, components considered important to maintaining SDC during a mode 5 condition.

As a minimum, this would include the SI l A or SI-l B control switches. Other components may be de.egnatea as " Peer Check" components as required by the SS/LSO.

, Overnressure Protection; These tags are fluorescent orange and will pump control switches as directed by the procedure.

In order to support the usage of the plastic labels DURING MODES 1,2 & 3, th

following tags)
guidelines are established for the "Out-of Normal (OON)" Plastic 1.

Switches: The label shall be used whenever any switch (control switch, SBM switch, etc.)in the Control Room is out of normal position during the performance of a surveillance test or OI. It is not expected that switches on equipment that is "normally" off or on, depending upon what the equipmen rotation schedule requires. For example:

A.

During theperformance ofOP-ST-SI-3008, you close the discharge valve or take the SI-2C HPSIpump CS topull-out. Both ofthese would require the use ofthe pink label and the position loggedin the "Out-ofPosition "

log.

B.

During normal equipment rotation, the "C" Main Feed Pump is started and the "A " Main Feed Pump is shutdown. Since this evolution is

, considered normal, and thepumps are started andstopped based upon the equipment rotation schedule, pink Out-of-Position labels shouldNOT be hung on either CS.

Q During a shutdown to mode 3, you are directed to take the Steam Dump L and Bypass Valve "Autoffnhibit" switch to " Inhibit". This wouldrequire hanging an OONposition tag on the CS.

Controllers: The small magnetic pink " dot" OON labels will be hung next to a controller whenever it is taken from " AUTO" control to " MANUAL" contro Normally, the pink dot will be magnetically attached next to the controller. (As 9/10/97, we do not have any Pink Dot OONtags. They are on order and should be available by 9/20i97).

A.

For example: You are going to take the 101XPressuri:er level controller to manual to support a surveillance test. This wouldrequire placing a pink dot "out-of-position " label next to the controller.

Peer Check Switches: Anytime a " Peer check" switch is taken out-of-position, a

, pink label shall be hung on the switch.

4.

Danger Tagged Components: An "out-of normal" plastic label will be hung on components that are danger tagged in the control room.

. Netify SS/LSO: The operator perfonning the test is responsible for hanging the bright pink labels on the CS or handle for the component andinforming the SS/LSO ofthisfact.

O 6.

Exceptions: The pink OON plastic labels do not need to be hung if the component being taken out ofits normal position is under the direct control of a licensed operator and is of short duration. For example:

A. During the performance ofPORYtesting, the PORV's are cycled open and then closed very quickly. The PORYcontrolswitches are " Peer Check" switches and would normally require an OONtag be installed. However, in this case, since the switches are under the direct control ofa LO and will be out ofthe normalpositionfor short time, the OONtags do not need to be hung. ,

The Shift Supervisor is allowed to exercise some discretion as to which control switches require OON labels and which don't. In all cases, the SS' will have final decision making authority.
