Test Monitor Program OV
1.0 PURPOSE
The purpose of the Test hionitor (Thi) is to provide an additional means of ensuring that Safety related Surveillance Tests (ST) and " Operating hstructions (OI)" tests (such as OI DO.)) are being completed exactly as written during the performance of the ST or 01.
The Test hionitor is expected to back up the operatc: conducting the ST/OI, thereby ensuring that steps are not missed nor steps taken that are outside the bounds established by the procedure.
2.0 ASSIONhiENT
The Shift Technical Advisor (STA) will be the Test hionitor under noimal conditions. In addition to the STA, Licensed Operations Department personnel can a t as a Thi provided they have been briefed on Thi expectations by the SS/LSO prior to asstuning duties as the Thi. Equipment Operators are not allowed to be Test hionitors.
Trainirm for the Thi rmition will require the Thi to be briefed by the SS on the expectations set forth in this procedure. Once briefed, the Thi is considered qualified to perfonn duties as a Thi.

3.0 EXPECTATIONS
A. The test monitor will have an additional copy of the ST/01 in hand prior to beginning the ST/Ol. The ST/Ol's that require the use of a Thi will be determined by the Operations Supervisor.
B. The Thi will review the ST/OI PRIOR to the performance of the test in order to fami!!arize him/herself with the test. Any questions that come up during this pre test review should be written in the margin next to the questionable step and resolved PRIOR to the performance of the test. The SS, LSO, LO or System Engineer should be consulted

to resolve the problem.
In addition to identifying problem areas within the body of the tert, the Thi will identify all annunciators that are expected to come in during the performance of the test. This practice will assist the Thi and the Operator when conducting the test by verifying the proper annunciator comes in to alarm at the correct time during the performance of the test.
C. Any FCS operating experience related to this test is expected to be reviewed prior to performance of the test. The Thi can obtain this information by running a query on the pd OPD-5-14 REY. O Page1of4

D. The FM is expected to follow the procedure exactly as written ~ % the Operator 4
performing the test and will check off steps as they are completec -
me same manner and same sequence as the Operator. The TM is required to concur with the pre requisites and precautions prior to commencing the test.
E. The Thi will not distract or otherwise hinder the Operator during the performance of the ST/Ol. He/she is expected to observe the test during performance and follow along with the operator. The one exception to this expectation is listed in "F" below.
F. If, during the performance of an ST/OI, the Thi observes any of the following activities or actions, THEN HE/SHE WILL IhihfEDIATELY BRING THIS TO THE ATTENTION OF THE OPERATOR AND STOP TIIE TEST.

The Operator perfor,ns or begins to perform a step out of sequence,
- The Operator performs or begins to-perform an action not stated in the procedure, The Operator manipulates a control incorrectly or begins to manipulate a control incorrectly (e.tr. CS, SDM switch, controller, etc.)
- Notes a step that is subject to interpretation or is unclear.
A simple statement of"STOP" should be sufficient to bring the operator's attention to the problem.
Once the problem is resolved to the satisfaction of the TM and the Operator, then the test i
may resume at the point it left off. If the problem cannot be resolved between the TM/ Operator, then the SS or LSO shall h cot.sulted for resolution. In all cases, the test WILL NOT PROCEED until resolution of the problem is achieved. In ALL cases Involving problem resolution issues, either the SS or the LSO's permission is j
required prior to proceeding with the test.
In the event the actions specified by a procedural step are not clear or are open to interpretation, then the TM will STOP the test and resolve the issue prior to proceeding.
The SS or the LSO shall be consulted in these cases and a procedure change shall be completed prior to re-commencing the test. In ALL cases involving laterpretation Issues, a procedure change must be generated as appropriate and either the SS or j
the LSO's permission obtained prior to proceeding with the test.
O. If, during the performance of an ST/OI, the TM or the Operator performing the test notes a procedural inadequacy that does not prevent successful performance of the test, but would enhance the procedure if corrected, THEN he/she will submit a procedure change to the ST/OI to correct the problem.

H. The TM is granted both the responsibility and authority to stop an ST/01 if he/she observes the test being conducted is NOT in accordance with the pmcedure or the actions -
specified in the procedure -k questionable.
I. Upon completion of the ST/OI, a critique sheet (attached) will be completed by the TM and the Operator. This critique sheet will list any areas for improvement that were
- identified during the performance of the test.
4 J. If the performance of an ST/OI will span two shifts, then the off going TM will turnover to the on-coming TM using the procedure as a turnover guide, included in this turnover will be, as a minimum:
- Current step in the procedure
- Problems associated with the procedure (if any)
- Procedure changes or recommendations made as a result of conducting the test (if any)
K. The TM will be secured upon completion of the procedural portions of the test. The TM's copy of the test may 1 e disarded at this time,
