fs Proceduret 1.0 PURPOSE i
1.1 To provide a procedure for establishing controlled l
access for tool accountability to the reactor cavity area when the reactor vessel head has been removed; to the pressurizer, primary and secondary sides of the 3
steam generators when their respective access ways have been opened; to the reactor coolant pump bays when their respective pump covers have been removed; and to 1
any other opening of the primary system when designated by the Supervisor-Maintenance as requiring Tool Accountability.
1.2 To minimize the possibility of introducing foreign materials into the reactor core and the primary system.
NOTE 1:
Foreign material will not be logged in and i
- out, i.e.,
trash, protective clothing, tape, rope, etc.
l NOTE 2:
All metal items i.e.,
bolts, screws, poles, etc. shall be monitored.
Any metal items to be permanently affixed in the tool accountability area l
shall be verified by tool accountability personnel.

2.0 RESPONSIBILITY 2.1 It is the responsibility of the Supervisor-Maintenance to ensure that tool accountability personnel are assigned whenever work is in progress in the reactor cavity with the reactor vessel head removed when the pressurizer access way is open and work is to be accomplished inside the pressurizer, or when the S/G accesses are open and work is to be accomplished in the S/G.
Tool accountability personnel are not required to be present if a lockable device or control system is t
used, i.e. posted signs.
2.2 It is the responsibility of each individual entering l
the reactor cavity area to comply with the requirements-of this Standing Order.
(2.2 also applies for the S/G and pressurizer when entering.)
2.3 It is the responsibility of each individual entering any tool accountability area to comply with this L
standing order.
In the case of a locked area or posted area an individual shall be assigned to tool I
accountability and be present prior to opening any area in question to provide tool accountability verification.

2.4 Maintenance personnel shall keep foreign material to a k--
minimum and shall remove any items immediately as i
requested by tool accountability personnel.
6 3.0 REQUIREMENTS 3.1 When maintenance is to be performed in the reactor cavity area, the pressurizer, or primary and secondary side of the steam generator, a person shall be designated as responsible for tool accountability during the maintenance.
The person assigned can work in conjunction with the assigned working crew.
t 3.2 The tool accountability personnel shall be designated l
during the pre-job briefing, and will ensure that all personnel entering the work area:
i a.
Are wearing coveralls with arm and leg openings i
taped.
Lab coats are D21 acceptable, b.
Have left all unnecessary loose items (pencils, watches, keys, coins, combs, jewelry, etc.)
outside the reactor cavity. area and steam generator areas.

Have secured necessary loose items by tape or t

Necessary items include film and security

l badges, dosimeters, safety / prescription glasses, hearing aids, and writing materials.
3.3 Tool accountability personnel shall a.
Conduct an initial inspection of the area to ensure that all loose items are properly secured i
or removed from the area, b.
Observe maintenance work activities in progress in the reactor cavity, pressurizer, steam generator, or other designated work area.
c.
During maintenance work observe the reactor cavity and other designated work areas to ensure that there are no loose tools or other items which l
could possibly enter the cavity or affected system if left unattended.
d.
At his/her option, maintain a log of tools in/out of the area to ensure their accountability.

The tool accountability personnel shall ensure when I
leaving the work area that one of the following actions l
have been taken:
t i
a.
Any opening to a system under maintsnance is l
sealed off with a cover (i.e. - gasket, material, o
plastic, etc.), and that cover is labeled for tool accountabili'y.
c b.
A physical barrier has been placed over the opening with locking devices or fasteners t
installed, c.
The maintenance work area has been roped off with signs posted to the affect of " Maintain Tool Accountability" in this area.
d.
Transfer responsibility for tool accountability to another crew worker in the work area and ensure this person is cognizant of this action.
3.5 When maintenance activities in the tool accountability i
area are completed, the tool accountability personnel shall note in the remarks section of the applicable maintenance procedure that tool accountability requirements have been observed, sign and date the i

4.0 ABNORMALITY REPORTING l
4.1 The Maintenance Supervisor shall be notified immediately of any significant abnormality.
