- recorded, checked and approved in a controlled manner.-
This Procedure is responsive to ANSI N45.2.11 and the Quality Assur-'
ance Plan.
x ;e 2.0 APPLICATION 2.1 Calcul ation s '
Calculations shall be formally included in the design package at.
the direction of the Design Engineer.
n The calculations are penformed on Form GSE-B-9-1 in a neat and orderly. manner so that assumptions, methods and results can be' readily checked.
The engineer performing the calculations.identi-fies each individual sheet, dates and initials it as the origina.

calculations indicating,' MR, title, revision and signatures of the e
preparer and checker.
Where diagrams, curves, and other inputs related to the calculation ~
l work are prepared on paper other than the standard calculation

- sheets, these pages are identified, initialed by.the originator, I
- dated, numbered, and referenced on the calculation sheets.
Each page shall indicate the total number of pages -in the calculation.
Reference shall be'made to the design drawing, specification and/or material list for which the calculation is performed.
l The calculation work shall be presented in the following order:
A.
Purpose - State the purpose of calculations.
B.
Assumptions - State the established conditions and assumptions.
C.
References - List all documents and computer codes referenced in the calculations.

Summary of Results - Statement of findings and identification of the result to be used in the design.

.Comouter Analyses Computer. analyses. shall. be checked by an engineer that is'exper.-
ienced in the use of the software which is utilized for the analy-sis.
-For " example, if T-PIPE is utilized, the checker must be experienced.with the.modeling techniques and theory of T-PIPE.
Each computer run should include the title, date, and page number
-on every sheet.. Listing of.the computer input shall be provided.
Final ' computer runs should _be identifiable by a unique number or code.
Machine generated program name and version should : appear-on each run and: the job I.D.
will be included with the. preparer's signature on.the calculation cover sheet.
2.3 C_QE Requirements 1
If. the calculations involved CQE equipment or components, the fol-
. lowing additional requirements should be followed:

CQE should be indicated on the calculation cover sheet for both the calculations and computer listing (if required).

Listing of the computer file identification shall be placed in
.the calculations with a brief description for each file, c)
The requirements for the computer runs are identical as stated in Section 2.2, Computer Analysis.
d)
Only the computer software which have bcra properly bench-marked and are subject to OPPD's QA requirements shvuld be used.
Use of p_on-benchmarked programs shall be justified in the calculations.
2.4 Review and Aonroval An engineer other than the performer of the calculation shall. check the results and indicate his concurrence by his signature on the calculation cover sheet.
The checker shall complete the checklist Calculations (Form B-9-3) and/or checklist - Computer Calcula-tions (Fcrm B-9-2) included in this procedure.

When calculations are modified to reflect changes in design' input, this f act shall.be recorded by a revision of the calculation sheets in accordance with Administrative Procedure A-11.
