SUPPLEMENTAL REPORT Initial notification of the subject deficiency was made to NRC-0IE, Region II, Inspector R. W. Wright on June 13, 1980. The final report was submitted on December 19, 1980. The first and second supplemental reports were submitted on May 21 and September 3, 1981, respectively. In accordance with 10 CFR 50.55(e) this letter constitutes our third supplemental report and finalizes TVA's response on this matter.

The shroud head and moisture separator asse.9tbly cleanup program has been successfully completed. GE has assured TVA that the assembly will operate as designed. The detailed results of the assembly cleanup program are located for NRC inspection in the BWR Sectior of the TVA Regulatory Staff, 400 Chestnut Street Tower II, Chattanooga, Tennessee.

A seven-day extension was discussed with and approved by NRC-0IE Inspector Austin Hardin on January 7, 1982; a five-day extension was discussed with Charles Burger on January 14, 1982.
