PHIPPS BEND NUCLEAR PLANT - REPORTABLE DEFICIENCY - OMITTED REBAR FROM RADWASTE BUILDING CONCRETE POUR - PBRD-50-553/81-22, -554/81-16 Initial notification of the subject deficiency was made to NRC-0IE, Region II, Inspector P. A. Taylor on August 14, 1981 as NCR PBNP-269.

4 ENCLOSURE PHIPPS BEND NUCLEAR PLANT OMITTED REBAR FROM RADWASTE BUILDING CONCRETE POUR PBRD-50-553/81-22, -554/81-16 10CFR50.55(e) REPORT NO. 1 (FINAL)
Description of Deficiency This deficiency concerns ten 11X11AW 23 1/2 (each face) reinroccing bars being onitted from a concrete pour in the radwaste buildine wall 3 at approximately 12' west of the vall 3 line at elevation (-16'

The probable cause of this deficiency sas failure by the appropriate personnel involved to correctly interpret the 'affected design drawings.
A similar deficiency, NCR PBNP-197, identified reinforcing bars that were omitted from concrete pours in_the shield wall of unit 1.
Safety Implication
'ad this particular condition remained uncorrected, the omitted rebar in the radwaste building (nonsafety-related structure) would not have adversely affected the safe operations of the plant. However, this deficiency appeared to have been a significant recurring condition and was repcrted on that banis.
Corrective Actions A.eview of omitted reinforcing bars over the past four months by Ouality Control-Civil and Quality Control-Materials personnel does not indicate that a trend adverse to safety is developing.
To prevent the future omission of reinforcing bar a commentary to Quality Control Instruction (QCI) C-207, " Reinforcing Steel Inspection," has been issued which is to provide a uniform interpretation to personnel involved in the installation / erection and quality control of reinforcing steel.
Construction of the Phipps Bend Nuclear Plant has been deferred. Although omission of the reinforcing bara presents no adverse safety impact, they still, however,will be added after construction has resumed. This aspect of the NCR will:not be closed internally to TVA until the reinforcing bars are added to the concrete pour in the radwaste building.
