FAILURE TO HYDROTEST MECHANICAL PENETRATION ASSEMBLIES -HTRD-50-518/81-22,

-519/81-18, -520/81-20, -521/81 PBRD-50-553/81-24, -554/81-18 Initial notification of the subject deficiency was made to NRC-0IE, Region II, Inspector P. A. Taylor on August 14, 1981 as NCR HTA QEB 8101, HTB QEB 8101, and PBN QEB 8102. Our first interim report was submitted on September 15, 1981. In compliance with paragraph 50.55(e) of 10 CFR Part 50, we are enclosing the final report on the subject deficiency. We consider 10 CFR Part 21 applicable to this nonconformance. As discussed with NRC-0IE Inspector Ross Butcher on February 5, 1982, TVA was granted an 18-day extension on the submittal of this report. Lescription of Deficiency --

This deficiency concerns the failure of General Electric, San Jose, California, to hydrotest the GE-NSSS-supplied flued head process line of the main steam penetrations before attaching the guard pipe supplied by Ametek. Paragraph NB6121 of ASME Section III requires that all mechanical welded joints of ASME Class I lines shall be left uninsulated and exposed for visual examination during hydrotesting. The guard pipe attached to the flued head process line prevents proper inspection of the welds during hydrotesting of the system. Adequate planning and coordination of responsibilities for the hydrotest of the flued head to main steam line weld was not accomplished before preparation of STRIDE senetration specification 400-09 Safety Implications Had this condition remained uncorrected, failure to hydrotest the flued head process line properly could result in inadequate welds not being identified. This could result in failures of these welds. The pressure boundary of the main steam line would not be maintained, thereby adversely affecting plant safety.

Corrective Action There are 24 of the subject penetrations affected by this deficiency.

Seven of these penetrations have been shipped to the Hartsville site without the necessary hydrotesting accomplished. Quality Control Inspection Reports (QCIRs) have been written against these seven penetrations indicating that the associated process piping has not been hydrotested.

V 1 These penetrations will be assigned by TVA to deferred units and the QCIR will be_ resolved before the penetration can be placed in operation. In addition, a memorandum from EN DES will be transmitted to the Hartsville j site on or before April 1, 1982, directing site personnel to effect the '

transfer of assignment.

In order to prevent recurrence of this deficiency, STRIDE specification 400-09, paragraph 6.2, has been revised to require the STRIDE mechanical penetration seller to perform an overpressure hydrostatic test on the main steam flued head process pipe before assembly with the guard pipe.

This revision ensures that the remaining 17 penetrations.have been or will be hydrostatically tested before shipment..
