Pursuant to the requirements of 10CFR Part 21, BIF is hereby notifying the Commission of a deviation which could create a substantial safety hazard.
On August 26, 1983, a Matrix actuator - Model 33122-SR 80 - was under test at Wyle Laboratories, Alabama, in accordance with the seismic requirements of IEEE 382-80, when a loosening of the mounting bolts occurred and the spring return cylinder began to separate from the center body or the actuator.
The test was terminated at this point and was considered to be a failure.
This test was conducted under a contract between Xomox Corporation, Cincinnati, 0 11, and Wyle Labor-atories.
Investigation of the failure and initiation of a corrective action program is being conducted by Xomox Corporation.
Results of this in-vestigation and corrective action recommendations are expected to be furnished by Xomox as soon as available.
Pertinent test data regarding the failure is not in BIF's possession at this time.
Butterfly valves shipped by BIP having this model actuator are:
