Dear Sir:

In accordance with the provisions of Section 2.201 of the NRC's " Rules of Practice," Part 2, Title 10, Code of Federal Regulations, Public Scr-vice Indiana offers the following information in response to the item of noncompliance 546/79-07-06; 547/79-07-06.

Item of Noncompliance (546/79-07-06; 547/79-07-06)

Nondestructive testing (pulse echo) of concrete structures by the Site Civil Contractor was initiated, although the following documentary evidence of conformance was either unavailable or inadequate:

1. Calibration records for the electronic instrunentation.
2. Verification records for Schmidt Hammer.
3. Personnel qualification records for pulse echo test technician.

Moreover, documented procedures or instructions for conducting, controlling and evaluating this nondestructive test activity were either inadequate or unavailable.

Corrective Action Taker And Results Achieved On 6-27-79 PSI requested persccnel certifications, appropriate equipment calibration records and a copy of the approved procedure from Newberg-Marble Hill. At that time, the personnel certifications, and appropriate MN 1512 080 ,.

'si PUBUC 2 SERVK:E Mr. Gaston Fiorelli Novembe. 1, 1979 INDIANA equipment calibration records were not available at the Marble Hill Site and the procedure, while available at the site, had not been subn itted to PSI for review.

PSI Quality Assurance Representatives instructed Newberg-Marble Hill to acquire the appropriate records and to retest any area tested before these records were on site and accepted. Testing was performed prior to PSI acceptance of these records solely for gathering preliminary data, and for equipment and process check out. All such teste were subsequently repeated. These conditions were documented by PSI Quality Assurance Personnel on Surveillance Inspection Reports and Field Corrective Action Requests. _This documentation is available at the Marble Hill Site for review.

Corrective Action To Prevent Recurrence Because the testing in question was performed on the basis of preliminary equipment and pre ess check out, the missing documentation was fully documented and controlled through PSI Surveillance and Corrective Action Reports, and because all o'.ficial data was obtained only after appropriate records and procedures were on site and approved , no further action to prevent recurrence is believed necessary.
