O ENCLOSURE PHIPPS BEND NUCLEAR PLANT UNITS 1 AND 2 IMPROPER INSPECTION RECORDS 10 CFR PART 50.55(e)

NCR M80-6 REPORT NO. 1 (INTERIM) v On May 20, 1980, TVA informed NRC-0IE, Region II, Inspector R. W. Wright of a potentially reportable condition unA r 10 CFR Part 50.55(e) regarding improper inspection records. This is the first interim report on this deficiency. TVA anticipates transmitting the next report on or before -

August 29, 1980.

Description of Deficiency During an audit at Phipps Bend conducted by TVA, it was observed that inspectors signing QA documents were not the persons actually conducting

  • the inspections. The specific area in which this practice was found was the Civil QC group. Apparently, a lead inspector was initialling the concrete placement form upon verbal confirmation from other inspectors, contrary to the requirements of QAPP-17, Revision 0, Quality Assurance Records.

Corrective Action The Civil QC inspection program is being reviewed to determine what changes are required to bring it into compliance with applicable requirements.

TVA will also investigate this concern at other construction sites.
