Facility:

Identification of Deficiency: Steam Generator Enclosures (2)

Improper Radiography initial Report:

On April 20, 1980, Mr. M. D. Hunt of NRC Region ll, Atlanta, Georgia, was noti fied of this deficiency by Mr. J. R. Wells of Duke Power Company, Charlotte, North Carolina, 28242.

Supolier and/or Component:

The steam generator enclosures were manufactured by Lakeside Bridge & Steel Company, 5300 N. 33rd Street, Milwaukee, Wisconsin, 53209, and initially radiographed by Wisconsin Industrial Testing, a subcontractor to Lakeside Bridge & Steel Company.

Description of Deficiency:

Penetrameters were improperly placed on film side of material rather than source side. Also, one area of weld metal was found to be undersized on each enclosure.

Analysis of Safety lmolication:

Improper NDE and inspection could allow welds not meeting design requirements to remain undetected.

Corrective Action:

Welds were radiographed in accordance with ASME Code. All welds found to be acceptable except one are& with less than minimum wall thickness. This weld was built up to design requirement and retested with satisfactory results.

Action was completed on May 17, 1980.

Duke Power Company management has visited the Lakeside facility in Milwaukee

+ to assure appropriate resources are applied to this deficiency. Duke Power will maintain close surveillance to prevent a recurrence and has instituted 100 percent film review-by Duke Power radiographic film reviewers prior to shipment.
