forwarded to you. \

%e subject investigation regards indications and fracture formation of the end coils of helical coil sprirus. These springs are used to supoly the closing force on safety valves at Duke Power's Catawba Nuclear Station.

We spring identification ntaber is #D34. The first fracture formation was found to propagate frm pre-service flaws in the ground eM surface. The second identified indications were fouM during subsequent magnetic particle examination. These were found to propagate fran the internally stamped spring identification number 4. -

As of this date, all subject springs have either undergone magnetic particle examination, or will, at the valve's next service cycle.

Any springs that display relevant indications will be replaced.

As an additional precaution, it has been reconmended that all valve spring end coils undergo re-magnetic particle examination to re-verify their suitability.

This event has been limited to the D34 springs supplied by ALCO Spring Industries,'Inc., of Chicago Heights, Illinois. Their examination will preclude any recurrence. All D34 AICO springs are installed at Duke Power's Catawba Nuclear Power Station.' '
