INSTALLED. THE VALVE WILL NOT FAIL CLOSED ON LOSS OF AIR SUPPLY UNLESS SOTH S# RINGS ARE INSTALLED.

IDENTICAL VALVE ACTURTORS WERE Suppl!ED ON MCSU!RE NUCLEAM STATION UNITS 1 AND 2 AND CATAWSA NUCLEAR STATION = UNIT 1 AND 2.

EACH NUCLEAR STATION HAS l

THREE VALVES, TWS 12 ACTUATORS TOTAL HAVE MEN SUPPLIED. WE ARE CERTAIN THAT THE MCSUIRE UNIT 1 AND CATAWBA UNIT 2 VALVES HAVE SOTH SSRINGS INSTALLED, SERIAL NUMSER 18789-9-1 ON CATAWSA UNIT TWO HAS MEN CORRECTED.

l ON THE OTMR ACTUATORS OUR RECORDS INDICATE SMINGS WEK ISSUED TO ASMMBLY AND TERE IS NOTHINS TO IMICATE THAT TMY MM NOT ASSEMSLED INTO THE UNITS.

SINGLE ISOLATED EVENT. WE HAVE NEVER ENCOUNTE ND A MISSING SPRINS IN AN ACTUATOR IN DISASSEMBLY OF ACTUATORS SY OUR FIELD K RVICE PERSONNEL NOR HAVE ANY MEN REPORTED MISSINS PRIOR TO THE DUKE EXPERIENCE.

THE DLME DOWER COM9ANY SHIPMENTS AM THE ONLY CLASS 1 SHIPMENTS WITH DUAL SPRING ACTUATORS.

P$tcntialSignificantDeficicncyReport-12/7/85 Page 2 cf 2 Duke Power recently disassembled the three (3) actuators on the Catawba Unit 2 Pressurizer Power Operated Relief Valves.

During disassembly of the actuator for valve Serial Number 18789-9-1 it was discovered that only one of the required two springs was installed in the actuator.

Valve fait close on loss of air is a safety related function for this valve.

Under design conditions, testing at Duke's Marshall Station has shown that valve will not fall close with only one spring.

Note that other two actuators which were disassembled had two springs each.

Duke Engineering records and Maintenance records indicate that discrepant actuator had not been disassembled at the site. CCI Field Service records agree with this.

This would indicate that the valve was shipped from CCI with only one spring.

Valve originally shipped from CCI around September,1981.

Prior to this time CCI drawings were revised to require two springs in the actuator.

Manufacturing Dis-position on the Engineering Order for any parts in progress was " rework".

Manu-facturing records for the valve in question indicate that both required springs were issued at the time of assembly.

No extra spring of the missing type are cu tuntly in stock.

Note that valve was tested for and successfully passed air failure mode during assembly testing at CCI.

This was, however, with no hydraulic loading on the plug.

Nike Osteen of Duke Power advised me of this discovery by telephone on 18 November 1983.

We have since provided the proper spring to Duke Power.

Duke Power has reported this deficiency to the NRC and reconnends that CCI do so under 10 CFR 21.
