[ /7 Introduction This report is subciitted to advise regarding the interim status and course of action required pursuant to MCAR #26 and CPCo NCR M-Ol-4-8-107.

Descriotion of Discrepancy ,

Refer to interim epm.c No. 1, dated January 8, 1979, for a description of discrepancy.

Probable Cause Furt'ter investigation by the Bechtel Supplier Quality Representative .(SQR) has indicated that the equipcent quality verification docu= cat packages are in order and that all crimping tools were properly calibrated and have proper calibration documentation on file. The investigation also indicated that the training records for nuclear production personnel were not on file ~

as required. Other than the improperly filed training records, the investigation ,

did not disclose any specific reason why the randon check by the SQR did not uncover any discrepancies. The training records were subs,equently retrieved and found to be acceptable.

The supplier provided his assessment of the probable cause, could not identify -

any specific program breakdown, and maintains that the discrepancies are of a random nature. It was suggested that the failures may be duc, in part, to the type of ter=ination lug that was used. Compression type, ring-tongue terminals with insulated ferrules were specified and provided. The insulation _

sheath covers the criep and consequently =akes visual inspection difficult. All of the suppliers current orders utilize uninsulated ferrules.

Corrective Action The supplier has initiated a 100% inspection program of all terminations on all current and future nucicar projects and has established a special training program for all nuclear production personnel. This should prevent reocurrence for any potential add-on orders for Midland.

.Page 2 With respect to the assemblies at the site, the supplier has perforced a preliminary invest'igation and verified that the discrepancies, exist. The o supplier has agreed to send representatives to the site in April to complete a comprehensive inspection of all penetrations designated for Class 1E circuits and selected penetrations designated for non-Class 1E circuits. They propose to inspect all of the #10, 14, 16 and. 20 gauge wires in all penetrations. Any crimps which are found to be defective will be replaced. All of the 350 MCM, 4/0, #2 and 6 gauge wire will be inspected in all of the penetrations which are designated for Class 1E' circuits. If the crimps in these penetrations pass a hand pull test, all remaining penetrations with like wire sizes will be con-sidered acceptable. If discrepancies are identified, the sample size vill be increased accordingly.

Safety Imolication Refer to interim report No. I dated January 8, 1979, for a discussion of safety 1.aplications.

Forecast Date of C -rective Action The corrective actions are scheduled for completion in April, 1979.
